Job Title: Primary Therapist - Registered NurseDepartment: Behavioral Health Location: 224 Alexander St, Rochester NY 14607 Hours Per Week: 40Schedule: 8:00AM-4:30PM Position Summary: Provide mental health treatment, with emphasis on pharmacotherapy, while allowing the patient the highest quality of life. Key Responsibilities: u2022 Demonstrates knowledge and ability to assess patientu2019s need for psychotropic medications appropriate to the chronological and development age of adjusts, children and adolescents. u2022 Accurately documents data obtained from psychotropic needs assessment of patient and of collaborations with primary care providers, psychiatrists, and other health team members. u2022 Plans and implements appropriate pharmacologic therapies interventions and evaluations of interventions of psychotropic medications prescribed including efficacy of medications. u2022 Updates MSR yearly and as needed as medical status changes. u2022 Assesses patientu2019s current mental status and ability to follow medical recommendations. u2022 Administers and stores psychotropic medications safely and in accordance with BHN policy. u2022 Monitors patient response to psychotropic medications. u2022 Provides psychotropic medication education and materials to patient and family members. u2022 Provides teaching and resources to multidisciplinary team members around psychotropic medications. Treatment Plan u2022 Formulates a comprehensive treatment plan integrating comprehensive information from the client, chart and other appropriate sources. u2022 Collaborates with patient and others to review objectives and outcomes of care. u2022 Coordinates care with primary medical providers and others in the community. u2022 Evaluates treatment plan in accordance with BHN policy and revises treatment plan based on most current assessment data. u2022 Develop discharge criteria for termination of the patient from treatment as part of the treatment process Implementation of Treatment Plan u2022 Engages in therapeutic relationships that focus on patientu2019s needs and provides a structured process for patient to reach the objectives outlined in the treatment plan. u2022 Revises interventions based on patientu2019s response. u2022 Documents all treatment contacts accurately and in accordance with BHN policy. u2022 Collaborates and communicates with primary care providers and multidisciplinary team as appropriate and documents contacts in patientu2019s record. Caseload Management u2022 Demonstrates ability to maintain a dynamic caseload. u2022 Demonstrates ability to determine medical necessity of care for patient. u2022 Articulates status of progress of patients on caseload, focus of treatment and plan for length of stay. u2022 Demonstrates ability to effectively and efficiently manage the indirect responsibility of patient care. u2022 Performs other duties as assigned. Minimum Qualifications: u2022 Associateu2019s degree in Nursing (ASN) or nursing diploma required, Bacheloru2019s degree in Nursing (BSN) preferred. u2022 2 years of experience as a Registered Nurse preferred u2022 Strong analytical, computer, and communication skills Required Licensure/Certification Skills: u2022 Current New York State Registered Nursing license EDUCATION: AS: Nursing (Required) LICENSES / CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA)American Heart Association (AHA)American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)New York State Education Department (NYSED)New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. PAY RANGE: $75,520.00 - $101,014.00 CITY: Rochester POSTAL CODE: 14607 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
