Outpatient Registered Nurse PACT

Summary The Staff Registered Nurse with the Outpatient Patient Aligned Care Team (PACT) uses the nursing process and evidence-based practice to collaborate with Veterans, Providers, Clerical/Clinical Associates, Family/Caregivers and Providers involved in providing care to Veterans and developing the patient-driven care plan. The PACT RN is responsible for collaborating with services internal and external to the VA to facilitate care transition in order to effectively meet the patients' needs. Responsibilities Major duties include but are not limited to the following: Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care outcomes by collaborating with members of the interdisciplinary team. Provides creative approaches to management of complex client care. Manages assigned panel(s), utilizes registries, and ensures appropriate evaluation and access is provided to patient panels. Provides health promotion, health education and coaching on wellness, disease prevention, and chronic care management. Collaborates in the development, implementation, evaluation, and revision of policies, procedures, and/or guidelines. Demonstrates performance and leadership that is broad enough to improve the care for the Primary Care Department and facility. Provides peers with formal or informal constructive feedback for improvement. Supports colleagues and other nurses through primary care knowledge sharing to provide safe, quality nursing care. Evaluates outpatient outcomes of evidence-based decisions and practice changes for primary care patients. Responsible for the documented PACT outcomes at the program or service level. Outcomes must be broad and complex.
