Abode Care PartnersWe are seeking an experienced ACO Program Manager to join our Value Based Operations team within BrightSpring’s Population Health Services organization. This role is strategically important to the organization as we expand our ACO capabilities and we need an experienced ACO Program Manager to join the team. We are looking for someone that has a proven track record in building and improving an ACO program and driving to exceptional outcomes, year over year. You will be at the intersection of operations, clinical model, finance, data analysis, and external partners.The ACO Program Manager is accountable for day-to-day execution of MSSP ACO requirements, is an internal subject matter expert and trusted cross-functional partner. This person will be motivated by our mission to deliver healthcare the way it should to vulnerable patient populations across the country. This person is comfortable rolling up their sleeves to dive into detail and equally comfortable at the 20,000 ft level. ACO program execution entails overseeing complex, cross-functional initiatives to support the overall success of the program(s). Some examples include:Develop, monitor, and maintain in-depth knowledge of ACO program rules and requirements, and translate those rules and requirements into business and operational executionBuild/revise program design, iterate to scale and improved outcomesLead ACO program implementation as the practice evolves ACO participationData reconciliation and analysis across multiple data sources creating the insight that teammates and leadership Identify opportunities for program improvement, intervention, issue escalation, etc. grounded in regular performance monitoringCollaboration with internal and external partners to review or resolve programmatic needs, may include file exchanges, audit compliance, roster and attributed lives review, physician credentialing This role will work with our VP VBO to ensure that all required program activities are completed, and that primary care is successful in a subset of our ACOsExecute on ACO program requirements including member/provider communications and policy and procedure developmentMonitor and summarize CMS communications and policies to identify notices, policy or regulation changes to ACO program that may impact Primary CarePrepare internal and external communication materials to support ACO operationsEngage with other team members including IT, sales, finance, and clinical to ensure completion of ACO program requirementsBuild trusting relationships with internal teams to influence and support successful performance in ACO programsProgram design and deployment of a performance management process to drive execution and target achievement for partnered network and employed providers including system-level target setting Identification and management of network curation opportunities Bachelor’s Degree preferred, preferably in health care administration, business or other healthcare related fieldBonus for Master degree in business administration, public health, or other related degree field3-5 years of ACO program management (MSSP, REACH) or related experienceHealthcare quality management and/or transformation experience in a primary care or other value based organizationUnderstanding of the value based care industry Knowledge of primary care operations, bonus if within LTC and AL patient populationsAnalytic skills to collect information from diverse sources and summarize the information and data to solve problemsStrong verbal and written communication skillsExcellent organizational and time management skillsProficient in systems including Microsoft Office, HERStrong analytical skill set, advanced Microsoft Excel skills, PowerBI/analytic platform Ability to work independently as well as part of a teamAbility to comply with HIPAA guidelines Ownership mentality that shows up as taking action, thinking creatively, and finding iterative ways to improveComfortable with ambiguity and change while simultaneously building new processes and revising existing onesVariable travel for training, shadowing and team meetings (quarterly at most)Abode Care Partners, an affiliate of BrightSpring Health Services, is a leading provider of integrated medical services, caring for individuals from post-hospitalization to home in various settings ranging from skilled nursing facilities, assisted living, independent living, group homes, and private homes.
