General Flexible role (full time and part-time options available)Established team of 1 Medical Director and 4 Advanced Practitioners Established practice serving the communityExperienced staff, both clinical and non-clinical, who know the patients and the communityFully integrated system with clinics and hospitals on EPIC EMR to improve care coordinationFull & part-time rolesClinic hours: Monday-Friday 8am-5pm4 day work weekNo nights, weekends, or holidaysPalliative Care services provided in patient's homes, clinic and/or nursing home settingAverage of 8-10 patients per day; no nights or weekends Shared call (telephone only) amongst the providers.
