As a designated regional referral center Trinity Regional Medical Center offers a wide range of primary and secondary care services. Trinity is accredited by DNV Healthcare Inc. and has achieved Pathway to Excellence designation by the American Nurses Credentialing Center. General Flexible role (full time and part-time options available)Established team of 1 Medical Director and 4 Advanced Practitioners Established practice serving the communityExperienced staff, both clinical and non-clinical, who know the patients and the communityFully integrated system with clinics and hospitals on EPIC EMR to improve care coordinationFull & part-time rolesClinic hours: Monday-Friday 8am-5pm4 day work weekNo nights, weekends, or holidaysPalliative Care services provided in patient's homes, clinic and/or nursing home settingAverage of 8-10 patients per day; no nights or weekends Shared call (telephone only) amongst the providers. Currently 1:17 - Triaged through MyNurseEvaluates, interprets, and coordinates information to make decisions and recommendations regarding patient carePatient-focused, friendly, and willing to collaborate in a team environmentExcellent communication (written and verbal), analytical and problem-solving skillsNurse Practitioner or Physician AssistantExperience in post-acute and/or long term care facility preferredPalliative Care certification preferred Ability to work as a team memberHold an unrestricted medical license in the state of Iowa Hold current and unrestricted state and federal authority to prescribe and administer medications as necessary or appropriate to the Advanced Practitioners field of practiceWe offerSalary based position with performance incentive Attractive incentive packageA competitive Total Rewards program offers benefits that align with your needs and priorities, no matter what stage of life you are in.
