Summary The Licensed Vocation Nurse provides high-quality, evidence-based care for a culturally diverse Veteran population; emphasizing patient centered care to veteran's ages 18 to 75 and above in the outpatient care setting. The LVN will work under the direction of the Surgical Specialty Clinic Providers to screen Veterans for chronic specialty diseases and to provide access to newly diagnosed Veterans with Surgical Specialty conditions and/or chronic diseases. Responsibilities The Licensed Vocational Nurse (LVN) performs nursing assignments in the Surgical Specialty Clinics and for patients as a member of the Bariatric, Ear, Nose & Throat (ENT), General, Gynecological, Neurosurgery, Oncology, Ophthalmology, Orthopedics, Plastic, Podiatry, Pre-operative, Urology, Vascular, Thoracic Clinic Teams. Under the direction of the Physician, Physician's Assistant, or Nurse Practitioner, she/he is responsible for providing direct and/or indirect patient care to the Bariatric, Ear, Nose & Throat (ENT), General, Gynecological, Neurosurgery, Oncology, Ophthalmology, Orthopedics, Plastic, Podiatry, Pre-operative, Urology, Vascular, Thoracic Clinic patients. She/he will have the knowledge, skill, and experience to carry out assignments including but not limited to data gathering, setting up/breaking down and assisting with diagnostic procedures and treatments. This LVN will take an active role in process improvement and quality monitoring. The LVN ensures Surgical Specialty Clinic providers are adequately supported during clinics and procedures and will serve as liaison between support services and Surgical Specialty Clinics. Conducts clinical reminders as appropriate for scheduled and unscheduled patients. Perform vitals including height and weight on all veterans with appointments. Notifies Attending/Resident/Case Manager of any abnormal findings. Use HYPERTENSION NON-PACT OUTPATIENT CLINIC GUIDELINES for any abnormal BP reading. Perform EKGs for emergent/urgent patients or pre-operative patients. Completes clinical documentation in the electronic medical record. Assist in the data collection (including physical, psychosocial, emotional, spiritual, cultural, and educational) for patients about to be and currently undergoing Surgical Specialty Clinic treatments. The LVN assists in screening of telephone calls and alerting providers of lab results and diagnostic tests for further review. Prepares and administers prescribed medications (oral, topical, subcutaneous, and/or, intramuscular) and performs treatments according to established policies/procedures; Observes for physical and/or emotional changes in patient's health condition, including responses to prescribed medications/treatments, and promptly and accurately documents identified changes, and reporting deviations from normal to the team RN, NP, or MD. Accurately and consistently identifies, documents, and reports subtle changes to veteran changes in condition. Uses treatment modalities and implements appropriate practices and procedures with complex patients. Utilizes skills to influence and communicate with difficult and challenging veterans and families. Demonstrates the ability to work to the capacity of his/her scope of practice and communicates effectively with the veteran, families, caregivers, professionals, and other supportive staff. Maintain, order and refresh stock and supplies required for ALL Surgical Specialty Clinics. Participates in the provision of care in the Surgical Specialty Clinics at the Loma Linda VA Medical Center, and Ambulatory Care Center, as appropriate and within the LVN Scope of Practice. Collaborate with Pharmacy services for medication supply. Screen patients in clinic and via telephone calls. Enters Encounter nurse clinic visits.
