Together, we are creating an environment where nurses are owners in the clinical decision- making process, are well connected, have open communication with their colleagues, feel supported to develop professionally, and understand the future of nursing at MultiCare.ResponsibilitiesYou will ensure the patient and family experiences seamless and safe transitions of care and engage with them as an integral and essential component of the care delivery processYou will advocate for patients and families, ensuring sensitivity to cultural and ethnic practices and beliefsYou will provide patient-centered education of disease processes and wellness opportunitiesYou will assist patients and families toward self-management of care including goal setting for management of chronic disease, improving medication adherence, creating a plan for behavioral change and coordinating community resources via nurse visit, telephone management, or other contacts, as neededYou will collaborate with leadership and the care team to identify opportunities, and develop a plan of action to promote appropriate utilization of resources across the continuumYou will serve as resource for staff, colleagues, physicians and advanced practice providers to positively impact CMS core measures and National Patient Safety Goals.RequirementsBachelor's degree in Nursing, or obtained within five (5) years from the initial date of hire; RN with 15+ years of service or internal candidates hired before January 1, 2021 may be considered in satisfaction of this requirement in lieu of an education agreementCurrent Registered Nurse licenses in Washington State or Multistate license endorsement (MLS)Master's degree in nursing preferredCertification as Certified Case Manager (CCMC), Accredited Case Manager (ACMA), Care Coordination and Transition Management (AAACN), Ambulatory Care Nursing (AAACN), or similar required or obtained within 3 years of hire (Employees hired prior to 8/1/2023 will have this requirement waived)Three (3) years of recent clinical hospital/ambulatory experienceKnowledge of care continuum (inpatient, ambulatory, post-acute, etc.) preferredAbout Auburn Medical CenterMultiCare Auburn Medical Center, serving South King County since 1921, is a 195-bed hospital with 58 behavioral health beds, a Family Birth Center, Level II NICU, Level III Trauma Center, MultiCare Regional Heart & Vascular Center, and advanced surgical services including robotic-assiste
