Responsibilities + Perform patient evaluations and develop individualized treatment plans. + Regularly reassess treatment plan effectiveness; attend staffing meetings and family conferences. + Conduct discharge evaluations, recommend home and community safety modifications, and provide referrals and educational resources. + Communicate patient needs and progress to the interdisciplinary team, physicians, patients, and families. + Collaborate across disciplines to coordinate care and improve patient outcomes. Qualifications + Degree in Physical Therapy from an accredited program. + Active Physical Therapist license in practicing state. + Certification in Basic Life Support or CPR as per state regulations. + At least one year of physical therapy experience is preferred. + Knowledge of rehabilitation techniques for complex neurological injuries. + Ability to lift 50 pounds; safely move/transfer patients up to 100 pounds. + Physical stamina: fine motor skills, visual acuity, extended standing/walking, pushing, pulling, bending, stooping, reaching, kneeling, and positioning. + Health screenings or exams may be required per location or state policies.
