Summary The Residential Rehabilitation Treatment Program (RRTP) is a 154-bed residential treatment program that consist of a 60 bed Domiciliary Care for Homeless Veterans(DCHV) program, a 40-bed Domiciliary Substance Abuse program (Dom SA), a 28-bed Posttraumatic Stress Disorder Domiciliary (PTSD Dom) and a 26-bed General Domiciliary. A Social Worker in RRTP works closely with an interdisciplinary team in coordinating care for the veteran. Responsibilities Total Rewards of a Allied Health Professional Duties of the Social Worker - Residential Rehabilitation Treatment Program include, but are not limited to: Establishes and maintains effective therapeutic relationships with Veterans/support systems. Works with Veterans/support system who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, financial, and psychosocial issues. Identifies high-risk needs and prioritizes case management services. Completes an appropriate (brief or comprehensive) psychosocial assessment to determine the psychosocial functioning and needs of Veterans and/or their support systems. Utilizes the assessment, in conjunction with the Veteran, to facilitate a treatment/action plan which maximizes the use of treatment programs/resources for attainment of the highest feasible level of independent functioning. Participates and collaborates with the interdisciplinary treatment team and with Veterans/support systems to develop and implement treatment goals and interventions. Utilizes medical and mental health diagnoses, disabilities, and treatment procedures, including acute, chronic and traumatic illnesses, and medical terminology. Provides Advanced Directive information and education including but not limited to the following: Durable Power of Attorney for Health Care, Living Will, Health Care Directive, and/or treatment preferences document to Veteran and families. Assists the Veteran in preparing such documents, as requested. Performs comprehensive suicide risk screenings and assessments and develops and integrates safety planning and safety goals into the, individualized treatment plan. Identifies and assesses risk, safety, supports and implements appropriate interventions/treatment for those who are experiencing abuse, neglect, exploitation or are experiencing domestic or intimate partner violence. Collaborates with Veterans/support systems and the interdisciplinary treatment team members to ensure that appropriate VA and Community referrals are completed in a timely manner. Provides consultation and education to Veterans/support systems regarding community resources, VA benefits and specialty programs, and advance directives. Provides consultation to other treatment team and staff members regarding psychosocial needs of Veterans/support systems the healthcare planning and compliance with treatment. Participate on committees, work groups, and task forces at the facility as deemed appropriate by the supervisor, Mental Health Services Assistant RRTP Chief and/or RRTP Chief, Deputy ACOS and/or ACOS, Mental Health Service. Assists with the orientation and mentoring of newly hired social workers and, serves as a field instructor for social work students, if interested and qualified. Work Schedule: Monday - Friday, 8:00 am - 4:30 pm, subject to change based on the needs of the facility. Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year) Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child. Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66.
