Summary The Psychosocial Rehabilitation Recovery Center Nurse role is to collaboratively manage, coordinate and provide health care to a defined group of high-risk patients with diagnosed severe mental illnesses and substance use disorders. Care is provided to patients with the goal of achieving quality patient care by improving clinical outcomes, increasing continuity of care, and minimizing health care costs. Responsibilities The Psychosocial Rehabilitation Recovery Center Nurse role is to collaboratively manage, coordinate and provide health care to a defined group of high-risk patients with diagnosed severe mental illnesses and substance use disorders. Care is provided to patients with the goal of achieving quality patient care by improving clinical outcomes, increasing continuity of care, and minimizing health care costs. Major duties include: The Psychosocial Rehabilitation Recovery Center Nurse functions within a nursing process framework for problem-solving and care facilitation: assessment, planning, implementation, and evaluation. Job duties are focused on achieving optimum mental and physical health and illness prevention. She/he provides psychiatric and mental health nursing care to patients with severe mental health illnesses, including complex psychosocial and behavioral issues along with coexisting medical illnesses all patients in the PRRC. The Psychosocial Rehabilitation Recovery Center Nurse is expected to perform data collection and analysis for the purpose of improving care to patient groups, maximizing utilization of resources, and enhancing the case management program. The nurse's primary commitment is to the patient, whether an individual, family, group, or community (ANA Code of Ethics for Nurses). The Psychosocial Rehabilitation and Recovery Center Nurse role is to collaboratively manage, coordinate and provide health care to a defined group of high-risk patients with diagnosed severe mental illnesses and substance use disorders. Care is provided to patients with the goal of achieving quality patient care by improving clinical outcomes, increasing continuity of care, and minimizing health care costs. Responsibility includes the utilization of general clinical nursing skills and knowledge in care coordination of veterans and education of veteran's/significant others as indicated. The Psychosocial Rehabilitation Recovery Center Nurse will adhere to all current directives of VHA, and meet current Joint Commission standards, Commission on Accreditation of Rehabilitation Facilities (CARF) accreditation standards, state licensure standards, and VA-OIG expectations. Assesses, plans, implements, evaluates, and documents and to promotes health and achievement of treatment goals and includes collaboration with the multidisciplinary team. Administers prescribes therapies using appropriate judgement; evaluation of therapies is persistent and timely. Utilizes assessment and screening tools to plan and implement care: the comprehensive bio-psychosocial assessment, treatment planning, and evaluation, skills development, counseling, crisis intervention, preventive health care, medication education/ administration of medication and procedures in accordance with Medical Center policy. Requirements Conditions of Employment U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
