Key Responsibilities: Screen, evaluate, and develop individualized treatment plans for patients with speech, language, cognitive, or swallowing challenges. Utilize standardized assessments and evidence-based practices to guide clinical interventions. Monitor patient progress, adjusting treatment strategies as needed, and document outcomes in compliance with all regulatory standards. Collaborate with patients, families, caregivers, and healthcare team members to ensure seamless care delivery. Provide counseling and education to support patient and family understanding of treatment goals. Write detailed discharge summaries and maintain accurate, compliant documentation. Participate in team meetings, trainings, and continuing education activities. Supervise and support CFY or student therapists when applicable. Adhere to ASHA, state licensure, and professional conduct standards. Required Skills: Valid ASHA certification and active state licensure as an SLP. Proven experience in clinical evaluation and treatment of communication and swallowing disorders. Strong knowledge of regulatory requirements, documentation, and evidence-based practices. Excellent communication, teamwork, and patient advocacy skills. Ability to adjust treatment plans based on patient progress and clinical findings. Nice to Have Skills: Experience working in long-term care or similar settings. Supervision experience with CFY or student speech-language pathologists. Bilingual abilities or additional language skills. Familiarity with telepractice or digital documentation tools. Preferred Education and Experience: Masters degree in Speech-Language Pathology or related field. Minimum of 1-2 years of relevant clinical experience in LTC or similar environments. Other Requirements: Ability to start promptly on January 26, 2026. Willingness to obtain necessary DME, supplies, and travel reimbursements as required. Must meet all licensure, certification, and regulatory standards.
