Summary The Specialty Care Licensed Vocational Nurse (LVN) provides high-quality, evidence-based care for a culturally diverse Veteran population; emphasizing patient centered care to veteran's ages 18 to 75 and above in the outpatient care setting. Responsibilities The Licensed Vocational Nurse (LVN/LPN) performs nursing assignments in the Medical Specialty Clinics and for patients as a member of the Medical Specialty Clinics and for patients as a member of the Administrative Services, Anticoagulation, Endocrinology, Physical Medicine and Rehabilitation (PM&R), Preventative Medicine (PM), and Rheumatology Clinic Teams. Under the direction of the Physician, Physician's Assistant, or Nurse Practitioner, she/he is responsible for providing direct and/or indirect patient care to the Administrative Services, Anticoagulation, Endocrinology, Physical Medicine and Rehabilitation (PM&R), Preventative Medicine (PM), and Rheumatology Clinic Teams patients. She/he will have the knowledge, skill and experience to carry out assignments including but not limited to data gathering, setting up/breaking down and assisting with diagnostic procedures and treatments when/if applicable in the respective areas. This LVN/LPN will take an active role in process improvement and quality monitoring. The LVN/LPN ensures Medical Specialty Clinic providers are adequately supported during clinics and procedures and will serve as liaison between support services and Medical Specialty Clinics. - Perform vitals and clinical reminders as appropriate for scheduled and unscheduled patients. - Perform EKGs for emergent/urgent patients. - Completes clinical documentation in the electronic medical record. - Assist in the data collection (including physical, psychosocial, emotional, spiritual, cultural and educational) for patients about to be and currently undergoing Medical Specialty Clinic treatments. - Notifies the clinic manager of problems and assists in problem resolution efforts as requested. - Participates in the education of patients regarding telehealth opportunities and facilitates that process for those patients interested in telehealth including but not limited to VA Video Connect (VVC). - Prepares and administers prescribed medications (oral, topical, subcutaneous, and/or, intramuscular) and performs treatments according to established policies/procedures; Observes for physical and/or emotional changes in patient's health condition, including responses to prescribed medications/treatments, and promptly and accurately documents identified changes, and reporting deviations from normal to the team RN, NP or MD. - Accurately and consistently identifies, documents and reports significant veteran changes in condition. - Has competence of encouraging and communicating with significant challenging psychological and physical behaviors to promote optimal functioning. - Demonstrates the ability to work to the capacity of his/her scope of practice and communicates effectively with the veteran, families, caregivers, professionals and other supportive staff. - Maintain, order and refresh stock and supplies required for Medical Specialty Clinics. - Participates in the provision of care in the Medical Specialty Clinics at the Loma Linda VA Medical Center, and Ambulatory Care Center, as appropriate and within the LVN/LPN Scope of Practice. - Collaborate with Pharmacy services for medication supply. - Screen patients in clinic and via telephone calls. - Ability in accurately performing Accu-cheks. - Demonstrates ability to use
