Inthis role, you will conduct in-person Specialized Mental HealthRehabilitation Facility (SMHRF) assessments to evaluate individualsneeds for services and supports.Assessments are conducted in-person within hospitals, psychiatricfacilities and, at times, correctional facilities. When work takes placewithin a correctional setting, candidates must be able to pass afacility-required background check.This position requires daily local travel (approximately 85%) throughoutCook County and Lake County and follows a Tuesday through Saturday workschedule.About the programMaximus partners with the State of Illinois Department of Healthcare andFamily Services (HFS) to manage the assessment process for PreadmissionScreening and Resident Review (PASRR) Level I screens and completingLevel II Serious Mental Illness (SMI) evaluations for individualsapplying to, or residing in, Medicaid-certified nursing facilities.Other services include Follow Up Visits, admissions support forSpecialized Mental Health Rehabilitation Facilities (SMHRF) andSupported Living Programs (SLP) settings and Colbert Dementia Review.Continues to expand assessment services in the state.Why Maximus?- Work/Life Balance Support - Flexibility tailored to your needs- Competitive Compensation -Bonuses based on performance included-?
