43622BR Extended Job Title: Recurrent Staff Employee Org Level 1: Texas Tech Unv Hlth Sci Ctr El Paso Position Description: A job involving an appointment for infrequent work where the employee is called in as needed. Social Worker provides education and counseling to families, individuals and groups. Works in collaboration with multidisciplinary team under the Department of Neurology Amyotrophic Lateral Sclerosis (ALS) multidisciplinary outpatient clinic to delivery quality care to ALS patients and families. Complies with required standards for medical/legal responsibilities, and serves as a resource to other allied and administrative staff. Work is performed under clinic conditions and requires a high degree of contact with patients, families and clinical group to help prolong the lives of ALS patients and improve their quality of life. Requisition ID: 43622BR Travel Required: Up to 25% Major/Essential Functions: + Conducted comprehensive social evaluations of patients, including verification of health insurance coverage, policy details, and accurate contact information. + Assessed patient needs related to health insurance, medical payments, home health aides, transportation needs, and available social support systems. + Coordinated clinical appointment communication, contacting patients or caregivers by phone or e-mail when patients were non-verbal. Performed pre-clinical calls to assess all patient needs prior to clinic visits. + Participated in pre-clinic meetings, communicating critical patient needs to ALS team providers to be addressed during clinic encounters. + Assist patients with health insurance and disability applications, financial assistance applications, and UMC/TTUHSC discount program applications. In addition, assisted patients with medication assistance programs and Sun Metro Lift transportation applications. + Connected patients with ALS associations to facilitate access to Durable Medical Equipment (BME/DME). Assist patients with coordination and completion of all orders generated in the ALS clinic to include: Durable Medical Equipment (DME/BME) Medications Home health services (physical therapy, occupational therapy, speech therapy) + Augmentative and Alterative Communication (AAC) speech devices Gastroenterology and radiology referrals for Percutaneous Endoscopic Gastrostomy (PEG) placement Referrals for tracheostomy Coordinated nutritionist referrals. + Maintain ongoing communication with home health providers and respiratory therapists to ensure continuity of care and promote high-quality patient care. + Coordinated noninvasive ventilation and respiratory support devices. + Provided urgent assistance for patients experiencing rapid clinical decline, including coordination of: Noninvasive ventilator orders Respiratory equipment orders Noninvasive ventilator setting prescriptions PEG tube placement referrals Palliative care referrals Hospital care referrals. + Facilitated communication among all members of the ALS multidisciplinary team, including physicians, occupational therapists, physical therapists, respiratory therapists, speech therapists, nutritionists, DME providers, and respiratory equipment providers. + Ensured accurate transmission of patient demographic and health insurance information required for timely access to services and external providers. + Remain current with all licensure, certifications, and mandatory compliance and training requirements for this position. + Adhere to all policies, procedures, and practices (Regent Rules, TTUS, HSECEP OPs, etc.). + Personally demonstrate, display and act in accordance with TTUHSC EP's Values (Service, Respect, Accountability, + Integrity, Advancement, and Teamwork).
