Responsibilities include: + Delivering direct physical therapy services with excellence + Evaluating functional needs and updating care plans accordingly + Designing customized, outcome-based therapy plans + Collaborating with family members and care teams + Guiding and supporting therapy assistants + Maintaining accurate documentation, goals, and progress notes + Acting as a community and clinical resource + Working in a physically active, fast-paced care environment + Safely lifting, transferring, and assisting patients as needed u2714ufe0f What Youu2019ll Need + Graduate of an APTA-accredited PT program + Unrestricted Indiana PT license + 1+ year of healthcare experience + Reliable vehicle, driveru2019s license, and current auto insurance + Ability to travel within the Mishawaka service region (up to 50%) + Ability to lift 50u2013100 lbs and meet physical demands ?
