Join us Job Summary and Qualifications u200bu200bThe Registered Nurse coordinates and delivers high quality, patient-centered care in accordance with the nature and specific requirements of the department,and consistent with the scope and standards of practice for the relevantstate andspecialty. In collaboration with medical providers and other members of the care team, the RN provides individualized, comprehensive, and compassionate care using established nursing models such as u201cAssess, Perform, Teach, and Manage.u201d The RN serves as an advocate forpatients/families/caregivers and models a commitment to the organizationu2019s vision/mission/values to support an unparalleled patient experience and clinical outcomes that contribute to overall departmental performance.u200b u200bAssesses patient condition during admission and during each shiftas scheduled and/or as needed, identifying and reporting any changes in patient status. u200bAssesses patient environment for any safety risks and/or non-compliance with regulatory standards. u200bAssesses patient pain regularly to promote effective pain management, including reassessments after appropriate intervention. u200bAssesses patient condition for signs and symptoms of abuse or neglect. Reports any indications of abuse or neglect to medical provider(s), nurse leader, and/or appropriate agency. u200bPerforms procedures, monitoring, or other functions as ordered by the medical provider(s). Documents the administration of care in the patient medical record in a timely and thorough manner. u200bPerforms the administration of prescribed medications. Monitors patient for therapeutic response. Notifies provider and intervenes as appropriate in the event of an unintended response to medication. u200bPerforms exceptional care by responding promptly to patient requests. Strives to anticipate patient needs and resolve them proactively. u200bTeaches patients/families/caregivers about patient medical condition, patient status, treatment plan, medications and possible side effects, and follow-up measures. Translates complex medical terminology to ensure complete understanding. u200bTeaches patients/families/caregivers about any non-medicinal follow-up measures, such as healthy diet and exercise, disease prevention, and/or other lifestyle changes. Prepares patients and families/caregivers for future self-management. u200bManages a care team, providing oversight and delegation to members as appropriate. u200bManages evidence-based practices and continuous process improvement initiatives to meet and exceed quality measures and enhance the exceptional care provided to patients. u200bManages the quality of care by collaboratingwith a multidisciplinary team to coordinate, evaluate, customize, and deliver a holistic patient treatment plan that promotes and supports recovery. u200bManages the continuation of care by servingas the interface between healthcare providers and community support systems who coordinate ongoing care needs for specialized patient groups.
