FTE: 1.000000 Standard Hours: 40.00 Shift: Shift 1 Full-time: Rolewill focus on hospital-based inpatient unit work. Job Summary: The RNCase Manager coordinates care and services of selected patientpopulations through the acute care episode and across the continuum tosupport an effective discharge plan. The RN Case Manager workscollaboratively with interdisciplinary teams both internal and externalto the organization to improve patient care by assuming a leadershiprole in driving desired clinical and financial outcomes througheffective utilization and monitoring of healthcare resources. The RNCase Manager utilizes standards, guidelines, and protocols for caredelivery and incorporates data to continuously improve care andoutcomes. EXPERIENCE DESCRIPTION: A minimum of 3 years of clinicalnursing is required. 5 years of clinical nursing and case managementexperience is preferred. Experience in case management field includingtransitional planning, outcomes management and assessment preferred.EDUCATION DESCRIPTION: Professional knowledge of nursing theory andpractice at a level normally acquired through completion of a minimum oftwo years education at an accredited School of Nursing in order to beeligible for licensure as a Registered Nurse is required. Bachelor'sdegree is preferred. TRAINING DESCRIPTION: Accredited case managercertification (ACM-RN) through the American Case Management Association(ACMA) preferred. SPECIAL SKILLS DESCRIPTION: General knowledge of payorindustry (Medicare, Medicaid, & Commercial insurance regulations),resource management, and evidenced based clinical practice is essential.Theoretical knowledge of the nursing and case management processes.Analytical skills necessary to assess patient's discharge needs anddevelop solutions to problems concerning patients' mental status orphysical well-being to ensure optimal patient outcomes. Advancedcommunication and interpersonal skills with all levels of internal andexternal customers including but not limited to medical staff, patientsand families, clinical personnel, support and technical staff, outsideagencies, and members of the community. Other skills needed includeexcellent written/verbal communication skills, critical thinking,creative problem solving, proficient organization and planning. Basiccomputer skills. Must be self-directed with ability to tolerate frequentinterruptions and a demanding workload. LICENSURE DESCRIPTION: Requirescurrent state of Wisconsin Registered Nurse License or a Multi-stateNursing License from a participating state in the NLC (Nurse LicensureCompact).
