Job Description Summary: The Senior Consultant Home Care operates at HomeReach Hospice as The Family Support Services Senior Consultant. It is a multi-level position that supports several areas in the OhioHealth Hospice House and the volunteer program. This position is responsible for coordination, supervision and training of hospice volunteers in accordance with Medicare Conditions of Participation. These areas, for volunteer support, include but are not limited to; OhioHealth Hospice House: front desk and Suite B desk, family and patient kitchen, stockroom, inpatient unit, bereavement activities, sewing and card group and so on. Volunteer coverage is also provided in homes and nursing homes. In addition to supporting our hospice volunteers, this specialist will assist our Philanthropy Advisor and Bereavement counselor at Hospice House in community outreach and projects. The Sr. Consultant will be asked to assist with a variety of projects to enhance the visitor experience at Hospice House. Time will be spent in the community giving talks and manning information tables at different events. This is a creative and flexible position that exists to support the ideals and mission of HomeReach Hospice. The Sr. Consultant will interact with families, patients, staff and providers in this work, attend IDT and meetings as required. Responsibilities And Duties: Driving is an essential function of the job. 60% Executes sales strategy targeted to meet customer needs and achieve growth. Identify and develop new referral sources, utilizing relationship-based sales skills/strategies. Maintain current referral base assuring that services needs are being met and continuing to strengthen relationships. Utilize effective time and territory management techniques. 20% Educates patients and families on OhioHealth services/products provided in home/community. Collaborate/coordinate with clinical/intake/admission teams to ensure smooth patient transition. 10% Maintains accurate records and produces sales reports in intervals as directed. Maintains competitive data file and referral source profiles. Prepares for and participates in all sales meetings and other meetings/events as needed. 10% Serve as all-inclusive resource for referral sources. Provide/coordinate education for professional groups, staff and community.
