Kettering Health Troy | Troy| Full Time |DaysGENERAL SUMMARYTheClinicalOncologyPharmacy Specialistwillworkalongside ouroncology providers andnursing staffto deliveroptimalcare to the patients at one of our four Kettering Health Network oncology locations(Kettering,Soin, Troy, Fort Hamilton).The pharmacistis responsible forcoordinating and delivering pharmacy services in compliance with all applicable policies, regulatory agency requirements, state and federal laws andparticipatingin the quality improvement process to ensure safe,effective,and efficient delivery of quality care.PRINCIPLERESPONSIBILITIESAND DUTIESOF THEONCOLOGY CLINICAL PHARMACY SPECIALIST:Performs job responsibilitiesin accordance withorganizational and departmental policies and procedures, performance improvementobjectivesand environmental/infection control standards. Developsand implementprocessestocomply withtheHFAP (Healthcare Facilities Accreditation Program)and regulatoryrequirements.Completestherapeutic monitoring, patient assessments, interpretation of lab values, and pharmacokinetic calculationsfor chemotherapy, immunotherapy,investigationalmedications,or supportive caremedications.Advises clinicians on drugselection, dosing, etc. based on current clinical guidelines and literature.Performs computer order entry inan accurateandtimelymanner.Reviews/updates medication profiles and iscognizantof patient charges.Review medication orders for therapeutic appropriateness, duplications, interactions, overalleffectiveness,and safety.Readily seeksclarification if necessarywhilemaintainingappropriate documentation.Identifiesneed foradditionalpathologytestingin a givendiagnosis to further supporttherapeutic decision makingAssistsin conversion of typical inpatient regimens/desensitizationsto outpatient protocolsper insurance requirementsAdvisesand provides recommendationsto KHN oncology practitionersregardingsupportive care, prophylaxis,empiric dose modifications, toxicityrelateddose adjustments, etc.Collaborates directly with physicians on quality improvement projectsWorks directly with patientassistanceteam onacquiringfree drug from various manufacturersfor patientsConsistently works andproblem solvesindependently andeffectively.Supports teamwork and cooperation with co-workers in daily activities. Readilyvolunteersassistanceto helpco-workers'complete tasks.Participates in nursing educationactivities.Identifiesopportunities for process improvementswith the goalto provideprompt,safeand efficient patient care.Actively promotes pharmacybyparticipatingin multidisciplinary in-services/meetings, individualized drug information,consults.Participates in the education/experiential training of pharmacy students and pharmacy residents.ActivelyassistinP&T initiatives through monitoring of documented interventions, review of potential formulary addition requests, andassessment ofalternativecost-effectivepharmaceuticals.Actively enhances personal growth and development byparticipatingin educational programs, literature searches,in-services,and meetings.Assistswithnew employee orientation,training,and evaluations.Duties performed may include one or more of the following core functions:Directly interacting with or caring forpatients;Directly interacting with or caring for human-subjects researchparticipants;Regularlymaintaining, modifying,releasingor similarly affecting patient records (including patient financial records); orRegularlymaintaining, modifying,releasingor similarly affecting human-subjects research records.Maintains a positive attitude, respects diversity, and works well withothers.Supervises the activities of the pharmacytechnician.Attends any infusion-related reactions andassistswith managing any acutesymptoms.Seeks out and applies new pharmaceutical concepts and information as available. Disseminates current information to support other professionalstaff.Compounds sterile products according to current guidelines (ASHP, USP, NIOSH).Adheres to safety requirements in the handling and preparation of chemotherapy and other hazardous agents.Pharmacist also mustmaintaincompetencies to compound any intravenous medications (i.e.immunotherapy, chemotherapy)Attend P&T or subcommittee meeting assigned by thesupervisorPRN.Assistand collaborate with pharmacy leadership and clinic team on developing and implementing new process orimprovements.Support the workflow to increaseoralprescription capturerate.Quality/Operational process improvement project PRN.Medication use evaluation or Drug Class review PRN.Evaluates/reconciles all current patient medications includingover the counterand herbal medication for drug interactions.Attend multidisciplinary teammeetingassigned by thesupervisor.Understands 340B operations and practices medication distribution in compliance withstandards.SKILLS / ABILITIES / COMPETENCIES REQUIREDComputer literacyEnglishproficiencyAbility to work independentlyand supervise othersEffective communication (both written and oral) and interpersonal skillsOrganizational ability with attention to detailAbility to handle multiple tasks simultaneouslyAbility to work in high-pressure situations.Ability to work well on a team.Ability to meet deadlines.Ability to pass area-specific competencies.LICENSESCurrent pharmacy licensure by the state of OhioMaintenance of continuing education as required by lawEDUCATIONGraduation from an accredited college of pharmacy (BS Pharmacy or Doctor of Pharmacy)EXPERIENCEMust meet one of the following requirements:PGY1 residency or 3 years of hospital experience with exposure to oncologyPreferred experiences:PGY2 oncology residencyBCOPcertificationWorked in oncology clinical practice in multidisciplinary teamExperience in creating treatment plans in Epic/BeaconExperience in performingmedication use evaluations, cost-savinginitiativesand/or leading quality improvement projectExperience in clinical researchFISCAL RESPONSIBILITYMinimizes drug wasteDispenses medications according to accepted standards of practice and evidenceDemonstrates competency andcompliancewith 340B regulationsKettering Health is a not-for-profit system of 13 medical centers and more than 120 outpatient facilities serving southwest Ohio.
