This is an excellent opportunity for a motivated, patientu2011focused RN who is passionate about delivering exceptional outpatient care, supporting care coordination, and enhancing the overall patient experience. Hybrid Schedule: Monday-Friday, 8am-4:30pm, up to two remote workdays each week About the Position: The Outpatient Nurse Coordinator is a Registered Nurse and key member of a highu2011volume pulmonary outpatient clinic care team. This role provides comprehensive oversight of outpatient care using independent judgment and continuous communication with patients, families, providers, and community resources. Responsibilities include patient education, medication management support, obtaining prior authorizations, and serving as a liaison between patients and providers. The Nurse Coordinator advocates for patients at all times and demonstrates a strong commitment to the community and the nursing profession. Responsibilities: + Engage in ongoing professional development and participate in shared governance and quality initiatives. + Provide leadership within the practice, supporting nursing and support staff and maintaining a safe, effective care environment. + Deliver holistic, patientu2011centered care, including triage, care coordination, education, and followu2011up. + Serve as primary nursing resource and liaison for patient caseload, coordinating with physicians and external services. + Build positive, collaborative relationships with patients, families, and all care team members. + Demonstrate strong critical thinking, clinical judgment, communication, and documentation skills. + Support department operations, safety, regulatory preparation, and practice improvements. + Educate patients, families, students, and new staff; may precept and assist with staff development. + Promote a compassionate, patientu2011focused experience and contribute to a culture of continuous improvement. + Minimum 3 years nursing experience. _BSN preferred._ + The individual must be able to demonstrate the knowledge and skills necessary to provide care and/or interact appropriately to the ages of the patients served by his/her assigned unit as specified below. + They must also demonstrate knowledge of the principles of growth and development over the life span and possess the ability to assess data reflective of the patients status and interpret the appropriate information needed to identify each patients requirements relative to his/her age-specific needs and to provide the care needs as described in the department policy and procedures. Licensure, Certifications, and Clearances: + Current licensure as a Registered Professional Nurse in practicing state. + UPMC approved national certification preferred. + Basic Life Support (BLS) OR Cardiopulmonary Resuscitation (CPR) within 30 days of hire + Registered Nurse (RN) Current licensure either in the state where the facility is located or, if the facility is in a state covered by the multistate Nursing Licensure Compact (NLC) agreement, a multistate license issued by a participating NLC state. Hires and current employees working on an out-of-state NLC license who later change their residency to the state where the facility is also located will have 60 days upon changing their residency to apply for licensure within that state.
