$45.00 / HourRN UAS Assessment Nurse: Full-Time Special Project (3 Months)Elderwood IPAElderwood IPA is seeking experienced Registered Nurses (RNs) to support a special community health assessment initiative.In this role, the RN Assessment Nurse conducts New York State Uniform Assessment System (UAS) community health assessments for members in their homes or virtually. These assessments help determine functional status, care needs, and eligibility for long-term services.This position is ideal for RNs who enjoy assessment-focused work, independent field-based practice, and supporting complex populations through thoughtful evaluation and documentation. Unlike bedside roles, this position focuses on comprehensive patient assessments, clinical evaluation, and care coordination insight that directly supports care management teams and member service planning.Special Project OpportunityThis role supports a full-time community health assessment project with Elderwood IPA for approximately three (3) months.Following the project, nurses may have the opportunity to transition into per diem RN opportunities through Elderwood Staffing Solutions, Elderwood’s internal float pool supporting facilities throughout Western New York. These opportunities provide flexible scheduling with the ability to pick up shifts ranging from occasional coverage to part-time or full-time hours. There may also be opportunity toWhile not required, this pathway offers the ability to continue working within the Elderwood network after the project concludes.Registered Nurses with experience in community health, long-term care, or MLTC assessment work are encouraged to apply and support this important community health initiative with Elderwood IPA.What You’ll Do as RN UAS Assessment Nurse with Elderwood IPA:Conduct Comprehensive Community Health AssessmentsPerform NYS UAS assessments in members’ homes or through virtual visitsEvaluate functional status, medical needs, behavioral health factors, and community supportsComplete required 6-month face-to-face reassessmentsClinical Evaluation & Care InsightIdentify changes in member condition, functional ability, and care needsCompare reassessment findings to baseline data to monitor health status and risk factorsRecognize potential safety concerns or health deterioration and escalate appropriatelyCollaboration & Care CoordinationCommunicate findings with care management teams, primary care providers, and interdisciplinary partnersProvide education to members and caregivers regarding chronic conditions, medications, and care planningDocumentation & ComplianceComplete detailed documentation in the electronic medical record and NYS UAS systemsEnsure assessments meet DOH, Medicaid, CMS, and regulatory requirementsMaintain compliance with HIPAA and Elderwood IPA policiesRN UAS Assessment Nurse:Current Registered Nurse (RN) license in New York State in good standingBSN, AAS, or Diploma in NursingMinimum three (3) years of nursing experienceAt least one year of experience working with frail or elderly populationsExperience completing NYS UAS assessments strongly preferredValid New York State driver’s licenseComfortable conducting in-home and virtual assessmentsExperience using EMR/EHR systemsThis position requires regular interaction with residents, coworkers, visitors, and/or supervisors.In order to ensure a safe work environment for residents, coworkers, visitors, and/or supervisors of the Company, and to permit unfettered communication between the employee and those residents, coworkers, visitors, and supervisors, this position requires that the employee be able to read, write, speak, and understand the English language at an intermediate or more advanced level.WE ARE AN EQUAL OPPORTUNITY EMPLOYER.
