Summary The LPN works in a medicine specialty clinic, in collaboration, with the registered nurse medical support assistant and medical providers. Together, with the patient and their caregiver support system, they work to manage the care of medically complex adult patients across the continuum. There are multiple medicine specialty outpatient locations throughout the medical center at University Drive, Heinz Division and the Monroeville CBOC. Responsibilities The LPN works in a medicine specialty clinic, in collaboration, with the registered nurse, medical support assistant and medical providers. Together, with the patient and their caregiver support system, they work to manage the care of medically complex adult patients across the continuum. Daily management of scheduled appointments. This is accomplished through assisting medical team with preventative care and/or chronic disease management by obtaining vital signs, offering patient education, completes screening questions for a variety of preventative and/or chronic disease measures, assists with managing immunizations or injections and all other ordered medications and assists with exams/procedures (pelvic exams, joint injection, wound care, Tele-retinal, Tele-derm, EKG's, Phlebotomy, bladder scans). Other responsibilities include assisting the medical team or other Interdisciplinary team members with panel management such as follow-up of lab and test results, patient education and care coordination. Is an active participant in planning, implementation and evaluation of care. Participates in daily Huddles and team meetings. Demonstrates, in interactions, the ability to communicate effectively and professionally with both internal and external customers. Is knowledgeable of and properly initiates nurse protocols and Standard Operating Procedures (SOP) to help deliver holistic care. Confers with LPN TL, RN Care Manager, PCP, or appropriate person to deliver care when needed. Uses motivational interviewing and health coaching to guide patients in achieving goals of health promotion and disease prevention. Checks daily quality control checks on testing and other equipment/supplies and restocks as needed. Informs the charge nurse/nurse manager of broken equipment that require repair. Recognizes and appropriately responds to breakage/malfunction or loss of equipment, safety hazards or supply deficiencies and reports to appropriate personnel for corrective action. Participates in performance/ quality improvement activities with the goal of improving patient care outcomes.
