You will report to our Chief Medical Offer for the Dallas, TX CenterWell market. Your Role & Responsibilities + You will provide comprehensive care to seniors following clinical standards. + You will collaborate daily with your care team to ensure coordinated, high-quality care. + You will manage referrals, hospital/SNF coordination, DME, and home health services. + You will document care with support from quality-based coders. + You will participate in on-call rotation and support growth plans at your center. + You will have experience providing value-based care. What We're Looking For Required: + Master's Degree in Nursing or completion of a PA program with board certification. + Active, unrestricted NP or PA license in the state of practice. + Must be bilingual (English/Spanish) + Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. The Federal Government will require applicants to take the Interagency Language Rating (ILR) test as it provides. Preferred: + Experience in value-based care, primary care or Medicare Advantage settings. + Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and Medicaid and Medicare numbers + Familiarity with HEDIS quality indicators and Medicare guidelines. #LI-KD2 Scheduled Weekly Hours 40 Pay Range The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc. $115,200 - $158,400 per year This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
