Job Title: Registered Nurse - Home Care Department: Hudson Home Care Location: ElderONE - Hudson Pace Center Hours Per Week: 40 Schedule: Monday - Friday, 8:00 AM - 4:30 PM SUMMARY As a Registered Nurse in Home Care, your exceptional ability to teach, counsel and empathize with each patient leads to an enhanced quality of life. RESPONSIBILITIES + Patient Care. Conduct initial assessments; establish plan of care and provide prescribed treatments and demonstrated nursing abilities; coordinate the delivery of 24-hour care; + Communication. Counsel and guide individuals and families towards self-help in recognition and solution of physical, emotional and environmental problems + Evaluation & Documentation. Conduct reassessment of each patient on a regular basis; maintain complete and accurate clinical documentation; supervise and serve as a resource to all home care staff REQUIRED QUALIFICATIONS + Nursing Diploma required + License and current registration to practice as Registered Professional Nurse in New York State + Current NYS Driveru2019s license PREFERRED QUALIFICATIONS + ASN preferred + Bachelors degree of Science in Nursing preferred + 1 year of experience working with a frail or elderly population preferred + Availability of Motor Vehicle EDUCATION: AS: Nursing (Required) LICENSES / CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA)American Heart Association (AHA)American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)New York State Education Department (NYSED)New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. PAY RANGE: $75,734.00 - $101,014.00 CITY: Rochester POSTAL CODE: 14617 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
