This role would be the perfect fit for: -Passionate professionals who are driven to create meaningful, life-changing patient outcomes -Clinical Licensure in the state of California Required (PT, OT, SLP, CTRS, LCSW, etc.) -Knowledgeable in functional neuro rehabilitation -Full-Time Career with Competitive Compensation and Comprehensive Benefits Responsibilities u00b7 Manages the provision of individual patient services and collaborates with clinical staff to develop rehabilitation plans. Coordinates the ongoing provision of rehabilitation services.u00b7 Formulates comprehensive, transdisciplinary rehabilitation treatment plans, clinical outcomes, goals/objectives and service delivery for referred patients in conjunction with the patient/care partner and the clinical team.u00b7 Completes CC evaluation and develops patient initial, progress and discharge reports and monitors the patientu2019s status on an ongoing basis. CCs are responsible for completion of patient clinical outcome tools based on diagnosis.u00b7 Monitors patient, family, physician and payer/case manager, referral source satisfaction with the program on a regular basis.u00b7 Leads interdisciplinary case conferences, team meetings, and communicates Treatment Plans/Progress Reports on a regular basis to the physicians, external case manager/payor and/or patient/family members.u00b7 Schedules/facilitates interdisciplinary case conferences with the clinical team, payers/case managers, physicians and family members.u00b7 Communicates regularly with patient, family/ care partner, payer/case manager and physician regarding patient progress and any important issues that arise.u00b7 Facilitates acquisition of necessary equipment to enable achievement of agreed upon functional goals (i.e. HMO, insurance carrier, etc.) with funding source.u00b7 Participates in medical provider meetings to report/resolve medical issues for person served. u00b7 Monitors projected hours of service to be delivered to the patient and reports hours/subsequent changes to the local leadership (Executive Director/Director of Clinical Management/Program Manager).u00b7 Negotiates with funding sources for extensions of service beyond initial authorizations.u00b7 Acquires and tracks orders from physician for all provided ongoing services after the initial orders are obtained.u00b7 Adheres to standards of the Company's Code of Conduct, policies, HIPAA and Corporate Compliance programs and appropriate professional practice standards and applicable state/federal laws.u00b7 Attends, participates in and/or conducts internal staff development programs, obtains continuing education as required by Company policy and regulations. Maintains and enhances clinical practice skills.u00b7 Performs other duties, as assigned. Qualifications u00b7 Minimum of Bacheloru2019s degree in Allied Health (OT, PT SLP), Registered Nurse, Human Services, orRehabilitation Counseling/Psychologyandu00b7 Maintains relevant professional licensure/registration/certification in the state of practice.u00b7 Current certification as Certified Brain Injury Specialist (CBIS) preferred.u00b7 Current BLS (Basic Life Support) First Aid and CPR certifications.u00b7 Minimum of two-yearu2019s experience in a rehabilitation setting with neurologically impaired patientsu00b7 Prior case management and/or insurance administration experience preferred.u00b7 Demonstrates excellent organization, problem solving, decision making and communication skills.u00b7 Organized and able to function under minimal supervision.u00b7 Has reliable transportation and able to travel to/from multiple assignments/locations on a daily basis.
