Summary The Advanced Practice Nurse (APN) Certified Nurse Practitioner (NP) practices in the Cardiology Service to provide a full range of services emphasizing quality, timeliness, and the advancement of the Veterans Affairs (VA) mission. The NP provides advanced clinical practice, consultation, and management. If you are looking to apply to the Physician Assistant-Cardiology position, please apply using this hyper-link: Responsibilities The Nurse Practitioner is a licensed independent practitioner with advanced education and national certification who provides evidence-based care for a population of patients. Major duties and responsibilities of the position: - Independently manages a panel of patients with acute and chronic health conditions with an emphasis on health promotion and disease prevention using evidence-based practice. - Develops and implements an individualized plan to achieve optimal population health and disease management. - Demonstrates leadership and works collaboratively with the interdisciplinary team. - Orders and interprets labs, tests, and diagnostic studies. - Prescribes pharmacologic and non-pharmacologic interventions. - Initiates referrals and consults to specialty services. - Provides education to patients and their families. - Assesses and adjusts the plan of care as needed to achieve optimal patient outcomes. - Exemplifies the ICARE core values of-Integrity, Commitment, Advocacy, Respect, and Excellence. - Demonstrates proficiency in management and diagnostic strategies for patients with ischemic and non-ischemic heart disease, heart failure, and advanced heart failure - Develops transitional and outpatient strategies to avoid preventable hospitalizations in patients recently discharged with heart disease and heart failure. - Inpatient management of Cardiology and Heart Failure consult patients will be conducted under the supervision of a Cardiology Physician. - Recognizes need for referrals of patients to subspeciality services within Cardiology requiring advanced therapies - Maintains knowledge of New York Heart Association functional classes, stages, and symptoms of heart failure. - Demonstrates knowledge and proficiency in heart failure management strategies and corresponding contraindications, including pharmacotherapy, testing and intervention. - Identifies clinical signs and symptoms related to end-stage heart failure and indications for referral to palliative care/hospice. - Works closely with patients, reinforcing education on diet/fluid intake, close monitoring of weights and vitals and labs, educations on signs and symptoms of CHF exacerbation, introducing GDMT, following home tele notes and tailoring meds in order to avoid readmissions. - Monitors outpatients to recognize failure to current therapy early and initiate and facilitate timely advanced therapy referral and intervention.
