As an RN on the Home Care Team, you will provide services to those living primarily in Cleveland - east, west and the downtown district. Further coverage will expand as far west as Vermillion. In this role, you will provideassessments, complextreatmentsand education to help avoid readmissions to the acute care settingand ensure comfort.This position offers a flexible schedule, allowing you tomaintaina healthy work-life balance, all while changing the lives of your patients and their loved ones every day.This is a PRN role. A caregiver in this position will work flexible day or night shifts, including 4-, 8-, or 12-hour options. Day shifts run between 8:00 a.m. and 8:30 p.m. or 9:00 a.m. and 9:30 p.m., while night shifts may range from 3:00 p.m. to 3:30 a.m. or overnight hours. Weekend coverage is required one shift per six-week period, with scheduling for one holiday per year.A caregiver who excels in this role will:Provide direct nursing carein accordance withestablished healthcare organization policies,procedures,and protocols.Implement andmonitorpatient care plans.Provide supervision to LPNs,HomeCareAidesand families/caregivers involved in the care of the patient.Performaccurateand complete admission assessments and reassessments within a specifiedtime frame.Assesspatientfor safety risks,abuseand neglect.Serve as a primary coordinator of all disciplines for well-coordinated patient care.Assess and document patients' pain usingappropriatepain scale and manage patients'pain then.Prioritize and document patient problems based on assessed needs and problems in collaboration with patients,familyand healthcare team.Maintain complete and up-to-date documentation and files at all timesregardingpatient condition and effectiveness and timeliness of care and interventions.Escalate change in patient condition to patient’s physician asappropriate.Minimum qualifications for the ideal future caregiver include:Graduate from an accredited school ofProfessionalNursingCurrent state licensure as a Registered Nurse (RN)Basic Life Support (BLS)Certification through the American Heart Association (AHA)or the American Red CrossMinimum one year current clinical experience as a registered nurseOne year of clinical experience as an RN in an acute medical/surgical settingValidDriver’sLicense(VDL) in Ohioand proof of automobile insurance with $100,000/$300,000 coveragePreferred qualifications for the ideal future caregiver include:Bachelor’sofScience inNursing (BSN)Cleveland Clinic Home Care Services is afull servicehome health agency dedicated tocontinuethe World-Class Cleveland Clinic care at home.
