Care Coordination Assistant (Vacancy) Print ( Apply ufeff Care Coordination Assistant (Vacancy) Salary See Position Description Location Varies by position Job Type Full-time Job Number 26-3111-V1 Department 50 Columbus Public Health Division Health Opening Date 03/19/2026 Closing Date 4/9/2026 11:59 PM Eastern + Description + Benefits + Questions Definition Conduct outreach, case management, health education, support and consultation to CPH Home Visiting clients for the purpose of breastfeeding initiation at birth and continuation through six months of age. Provide breastfeeding education and coaching trainings on a regular basis for all home visitation staff. Conduct outreach activities to educate potential clients on breastfeeding services available. Act as a liaison with WIC staff and services to ensure continuity of care for MCH Home Visitation clients. Performs related duties as required. Pay Range: $22.41 - $33.17 Preferred Qualifications: Community Health Worker, CLC or IBCLC certification preferred. Prior experience working with pregnant women, families, or children desired; experience and willingness to work with culturally diverse populations. Basic computer skills knowledge preferred. Possession of a valid driveru2019s license and reliable transportation. Examples of Work (Any one position may not include all of the duties listed, nor do the examples cover all of the duties that may be performed.) Guides consumers through different social services and the healthcare system; Makes preliminary consumer needs recommendations; Provides short-term case management services to consumers enrolled in specific programs; Reviews eligibility requirements of social and health service programs in order to match prospective consumers with needed services; Facilitates the processing of applications for new consumers and volunteer guardians by assisting with completion of forms, obtaining the necessary documentation and information, and following up with appropriate action; Resolves barriers that may impact the customer receiving care from social and health programs; Consults with family members, guardians and health care, social services, and court officials to obtain additional information about consumers; Arranges for consumers to receive assistance; makes appointments for consumers at various service or government agencies; Maintains and revises a detailed directory of social services available in the community, relevant agencies, eligibility requirements for service, and agency locations; Uses computer programs to prepare and compile reports and records of consumers interviewed and assisted; Determines whether assistance arranged for consumers was helpful and prepares summaries on the utility of services rendered; Contacts new social service agencies and local programs to determine scope of services offered; Attends community events and performs related outreach activities to engage potential customers; Maintains relationships with established partners and served communities; Makes presentations to civic groups about programs and services; May communicate with individuals who have limited English language skills; May be assigned to serve as a member of an Incident Command System (ICS) Team, or a similar public health response team which may include the conduct of operations on a 24/7 basis at remote locations. Assignment may include serving as a department and/or community incident commander or on the incident command team. Minimum Qualifications Possession of an associate's degree in one of the social or health care sciences or two (2) years of college with a major in one (1) of the social or health care sciences. Substitution(s): Two (2) years of experience assisting individuals with accessing healthcare, social, or personal services may be substituted for the educational requirement on a year-for-year basis. Some positions may require the possession of a driveru2019s license. Some positions may require certification as Community Health Worker by the State of Ohio Board of Nursing.
