SUMMARY As a Community Health Nurse, you will focus on providing nursing interventions, patient education, and care management to a variety of clients in our community. This promotes good health and one on one patient care in the comfort of their own home. STATUS: Per-Diem LOCATION: Rochester Regional Health Home Care - Monroe Ave DEPARTMENT: HHC CHHA CENTRAL MONROE NURSING SCHEDULE: Per-Diem, Days ATTRIBUTES + Compassionate, independent ,and patient focused + Comprehensive nursing assessments and documentation, decisive judgment and professional behavior + Two (2) years experience in home care preferred + Bilingual preferred + At least one year of nursing experience RESPONSIBILITIES + Patient Care & Service. Provide comprehensive assessments of the bio-psycho-social needs of acutely ill clients in their home; perform skilled nursing care and prescribed treatments to clients based on MD orders; provide client and family education in accordance with client assessment and plan of care + Planning. Develop and document individualized care plans customized for each patientu2019s unique needs, with support from the interdisciplinary health team as needed; maintain effective communication to convey patient health status, treatment plans and progress + Communication. Accurately and completely document client care; document and implement physician orders per D.O.H. and program standards + Compliance. Adhere to required department and system protocols, regulations (local, state, federal) and education requirements REQUIRED QUALIFICATIONS + Diploma or Associateu2019s Degree in Nursing required EDUCATION: AS: Nursing (Required) LICENSES / CERTIFICATIONS: BLS - Basic Life Support - American Heart Association (AHA)American Heart Association (AHA), RN - Registered Nurse - New York State Education Department (NYSED)New York State Education Department (NYSED) PHYSICAL REQUIREMENTS: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting. PAY RANGE: $75,733.00 - $101,014.00 CITY: Rochester POSTAL CODE: 14607 The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
