Salary Range $24.00 - $26.00 Hourly Position Type Full Time Education Level Bachelors POSITION OVERVIEW Residential Case Manager Our Residential Case Manager will work out of our Albany, NY campus and will beu00a0responsible for supporting clients in the Residential System whileu00a0performingu00a0duties in collaboration with a clinician. u00a0 ESSENTIAL DUTIES AND RESPONSIBILITIES + Arrange and attend home visits + Prepare transition plans + Coordinate Independent Living Services + Act as the primary contact between Schools and Residential + Coach family visits + Complete various assessments, including FASP and SPR + Complete accurate case notes and timely input the notes into Connections database + Provide transportation for clients + Maintain contact with DSS and family + Function as a contributing member of the interdisciplinary treatment team + Attend dorm meetings + Attend treatment review meetings u00a0 EDUCATION and/or EXPERIENCE Requires a Bachelors degree or commensurate level of education and related experience. Experience with individuals in care systems preferred. Drivers license is required for this position.
