Key Responsibilities: + Provide daily clinical oversight of patient care services. + Coordinate referrals and admissions with internal and external sources. + Evaluate patient needs and agency capabilities before admission. + Supervise care planning, partner assignments, and care delivery. + Ensure compliance with regulatory standards and agency policies. + Mentor and coach clinical staff to promote morale and retention. + Utilize MatrixCare and Strategic Healthcare Partner systems for workflow and reporting.
