Thisposition serves as the Health Centers lead specialist in insuranceenrollment and resource navigation, proactively identifying and removingfinancial and social barriers that impact access to care.The Coordinator provides hands-on assistance with Medicaid, Marketplace,and SNAP applications; educates patients on coverage options and renewalrequirements; and responds to eligibility changes, denials, and coverageinterruptions to prevent gaps in care. In addition, this role leadsSocial Determinants of Health (SDOH) screening and resource coordinationefforts, partnering closely with medical, behavioral health, andoperations teams to deliver whole-person, patient-centered care.This position is ideal for a detail-oriented, mission-drivenprofessional who is passionate about reducing disparities and supportinghistorically under-resourced communities through direct service,advocacy, and systems navigation.KEY RESPONSIBILITIES:Screening Patience for Insurance Coverage- Proactively screens all new and existing patients to assess insurance status, eligibility, and gaps in coverage prior to or during clinic visits.- Educates patients on available coverage options, benefits, limitations, and renewal timelines to ensure informed decision-making.- Collaborates closely with all Health Center teams to ensure accurate documentation and continuity of care.- Reactively troubleshoots coverage issues, denials, lapses, or changes in patient circumstances that may impact access to services.Medicaid Applications- Proactively identifies patients who may be eligible for Medicaid through sliding scale application which includes- income, household, or life-event screenings and initiates application processes.- Provides one-on-one, detail-oriented assistance to complete applications, gather documentation, and submit materials accurately and timely.- Communicates clearly with patients about application status, required follow-ups, renewals, and next steps to reduce confusion and delays.- Reactively addresses application denials, requests for additional information, or coverage interruptions by coordinating with state agencies and internal teams.Marketplace Insurance Applications- Proactively assesses eligibility for Marketplace plans for patients who do not qualify for Medicaid and provides plan comparisons tailored to patient needs.- Educates patients on premiums, subsidies, deductibles, enrollment periods, and coverage differences in a clear, patient-friendly manner.- Works collaboratively with care teams to align insurance selections with anticipated healthcare needs and services.- Reactively supports patients during special enrollment periods, life changes, or plan-related challenges to maintain uninterrupted care.Supplemental Nutrition Assistance Program (SNAP) Applications- Proactively screens patients for food insecurity and eligibility for SNAP as part of routine SDOH assessments.- Assists patients in completing SNAP applications, recertifications, and documentation with sensitivity and confidentiality.- Educates patients on program benefits, usage, renewal timelines, and reporting requirements to support long-term stability.- Reactively intervenes when benefits are delayed, denied, or i nterrupted, connecting patients to emergency food resources as needed.Lead Social Determinants of Health (SDOH) Challenges- Proactively identifies SDOH barriers such as housing instability, transportation needs, food access, employment challenges, and financial strain through patient screening and outreach. Uses the screener to connect with patients during their visit.- Develops trusting, person-centered relationships to understand patient needs, preferences, and goals beyond clinical care.- Coordinates closely with providers, behavioral health, case management, and community partners to create tailored care and resource plans.- Reactively responds to urgent or evolving SDOH needs by connecting patients to appropriate internal services and external community resources.Medical Records ManagementManage the medical records process, including but not limited toreviewing incoming medical records requests, fulfilling requestsappropriately and in a timely manner, and ensuring accuracy andcompliance.Coordinate and communicate with internal teams and external facilitiesto resolve issues, obtain missing information, and supportproblem-solving related to medical records.Track
