Summary The Patient Aligned Care Team (PACT) Outpatient Staff Registered Nurse (RN) is responsible and accountable for all elements of the nursing process when providing and/or supervising direct patient care. Assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction. This position reports to the Associate Director of Patient Care Services (ADPCS). Responsibilities The Cincinnati VA Medical Center (CVAMC) is a two-division campus located in Cincinnati, OH, and Fort Thomas, KY serving 15 counties in Southwest OH, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH. The CVAMC also supports two off-campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division. Duties include, but are not limited to: This nurse will support colleagues through knowledge sharing to provide safe, quality outpatient nursing care. This Nurse provides health promotion, health education, and coaching on wellness, disease prevention, and chronic care management. This nurse integrates outpatient nursing knowledge, skills, abilities, and judgment, and is self-directed in goal setting for managing complex PACT situations. This nurse will demonstrate the ability to cope with and manage competing priorities. This nurse provides gender-specific direct veteran care and care coordination, as pertains to Veterans and their care with the primary Care Department. Evaluates outpatient outcomes of evidence-based decisions and practice changes for primary care individuals, groups, and populations. Administers medications and procedures per established polices and guidelines. VA offers a comprehensive total rewards package: VA Nurse Total Rewards.
