Summary The position is responsible for the delivery of Whole Health Coaching and skill development of the Cincinnati VA Medical Center's Whole Health Coach program, including provision of care through coaching, promotion, education regarding Whole Health Coaching to Veterans and staff. The position is integral to supporting Whole Health Coaches and Veterans in navigating the Whole Health Journey, specifically through the Pathway, wellbeing and Whole Health clinical care elements of Whole Health care. Responsibilities The Cincinnati VA Medical Center is a two-division campus located in Cincinnati, Ohio and Fort Thomas, Kentucky serving 15 counties in Southwest Ohio, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH. Cincinnati VAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division. Duties Include: Contact eligible participants for enrollment and explain the health coaching process. Provides health coaching for Veterans in order to reduce or eliminate high risk behaviors while increasing healthy ones. Conducts interventions for Veterans using one on one, couple or group modalities in outpatient and inpatient settings. Conducts Whole Health facilitated groups' participate in MOVE, smoking cessation, diabetes management, hypertension clinics, mental health and other inpatient and outpatient groups Provides follow-up care through telephone clinics or secure messaging between face-to-face appointments. Coordinates care with Veterans' treatment providers, both primary and specialty care, to ensure identified goals are consistent with best practices for treatment and management of chronic illness. Reviews and develops treatment plans for/with Veterans. Serves on healthcare system, VISN, and/or national VA committees or task forces specifically relating to Whole Health or Health Coaching. Assists with data management analyses, special studies and reports for use by management for budget preparation, workload projections, and quality assurance and utilization reviews. Provides training and troubleshooting for other health coaches in health coaching administrative processes and associated supporting issues. Conducts outreach activities to inform Veterans and family members of available programs Promotion Potential: The selectee may be promoted to the full performance level without further competition when all regulatory, qualification, and performance requirements are met. Selection at a lower grade level does not guarantee promotion to the full performance level. Work Schedule: Monday-Friday 8:00am-4:30pm Telework: Not Eligible Position Description/PD#: Health and Wellness Coach/PD99325-S Permanent Change of Station (PCS): Not Authorized Physical Requirements: While some of the work is sedentary including researching, writing, program planning and data analysis, the position will have periods of extensive walking and movement throughout the healthcare system, prolonged standing, and transporting light to medium weight objects. The Whole Health Coach will also be required to travel between sites (CBOCs and outlying clinics) encompassed in the healthcare system. The work is conducted in a medical center environment and involves risk of exposure to a variety of contagious diseases. The work area is adequately lighted, heated, air conditioned, and ventilated. Requirements Conditions of Employment You must be a U.S. Citizen to apply for this job.
