Purpose of Position Providecomprehensive primary care services for Access patients in acollaborative effort with all care team members. II. Essential Job Functions/Duties Perform,order, interpret and/or analyze patient histories, medical records,various medical reports and examination information to diagnosepatients' condition and choose treatment options. Prescribe oradminister treatment, therapy, medication, vaccination, and othermedical care to treat or prevent illness, disease, or injury incompliance with medical standards, laws and regulations. Patient caremay include hospital rounds for some medical patients, prenatal care inthe clinic and obstetrical services in the hospital. Collect, record andmaintain patient information within the electronic medical record.Monitor patients' conditions and progress and reevaluate treatments asnecessary. Explain procedures and discuss test results or prescribedtreatments with patients. Advise patients concerning diet, activity, andhygiene and disease prevention. Refer patients to medical specialists orother practitioners when necessary. Maintain shared goals, mutualrespect and trust with all care members to include nurses, pharmacists,psychologists, and other health care providers. Direct and coordinatepatient care activities of supporting medical staff. Keep up-to-datetechnically and apply new knowledge to the job. Provide medical guidanceand expert advice to management or other groups within Access. Activeand constructive participation in care team development andprocess/quality improvement projects. Effective communication with allcare team members based on Provider Expectations outlined by the MedicalDirector. Coordinate patient care across multiple settings. Workcollaboratively community partners to provide quality patient care.Works effectively on a team and communicate respectfully with patientsand all staff based on shared goals and mutual respect. This includesdemonstrating adaptability to continuous efforts to improve patient careand willingness to be an active care team member engaged in ongoingprocess and quality improvement projects within the framework of thePatient-Centered Medical Home. Honors patients right to privacy andconfidentiality. Adheres to strict patient, customer and businessconfidentiality standards. IV. Licensed Medical Doctor orDoctor of Osteopathic in Wisconsin or eligible for license in Wisconsin.2. Board certified (or board eligible) in Family Practice with orwithout Obstetrics. 3. Insurable for liability coverage as a physician.4. Evidence of continuing education, as required. 5. Experience withsupervising, motivating and directing medical staff. PreferredQualifications: 1. Experience working with mid-level providers. 2.Experience working with diverse patient population or working within acommunity health center. 3. Fluency in English and Spanish VII.
