Our Company Adoration Home Health and Hospice Overview The Patient Services Coordinator (Clinical) is responsible for scheduling home visits for field staff and coordinating patient care under the direction of the Clinical Manager. This role ensures optimal utilization of clinical resources to support care center capacity and adherence to utilization guidelines, while serving as a liaison between field staff, patients, and the Clinical Manager for scheduling and communication of patient services. Responsibilities u2022 Schedule patient assessment visits, including Oasis visits, in a timely manner. u2022 Prepare weekly clinical schedules and review daily to identify capacity opportunities and ensure productivity standards. u2022 Assign clinicians to geographic areas efficiently, matching skills to care needs and optimizing capacity. u2022 Reschedule missed visits within the current week or escalate to Clinical Manager. u2022 Collaborate with field clinicians to ensure all ordered patient visits are scheduled and completed. u2022 Manage scheduling-related calls from patients and staff; refer clinical or performance issues to Clinical Manager. u2022 Monitor hospitalized patients and ensure appropriate team follow-up. u2022 Complete schedules including required orders and services (e.g., lab draws, supervisory visits, add-on visits). u2022 Support Clinical Manager in timely communication with patients, clinicians, referral sources, BD team, and office staff. u2022 Run applicable reports and take appropriate workflow actions. u2022 Assist with patient transfers between care centers and/or Hospice services. u2022 Maintain supply closet, ensuring supplies are current and properly packaged; coordinate field supply needs. u2022 Perform referral and intake duties in absence of Intake Coordinator. u2022 Obtain and document verbal orders for patients in accordance with licensure requirements. Qualifications + Associate degree in nursing (ADN) or Bachelor of Science in Nursing (BSN) required + Minimum 1 year administrative experience in a healthcare setting + Scheduling experience in healthcare is preferred + Current unencumbered license to practice as a Licensed Piratical Nurse (LPN), Licensed Vocational Nurse (LVN), or Registered Nurse (RN) About our Line of Business Adoration Home Health and Hospice, an affiliate of BrightSpring Health Services, provides quality and compassionate services in the comfort of home, providing support for patients, families, and caregivers in their time of need.
