JOB REQUIREMENTS: Your Role The Behavioral Health Care ManagementClinician, Senior helps members with behavioral health needs navigatethe health care system for linkages to behavioral health providers,treatment, and programs. The Behavioral Health Care Manager will offersupport in assessing members' emotional and psychological well-beingand providing resource coordination, crisis intervention, substanceabuse, and with any behavioral or mental health referrals. TheBehavioral Health Care Manager will be available to physicians and nursecare managers as a consultant for patients with complex psychosocialneeds. The position requires an intensive focus on crisis interventionand counseling, problem-solving and conflict resolution, patient andfamily management, interdisciplinary collaboration, psychosocialassessments, education, advocacy, and community resource linkages. YourKnowledge and Experience Requires a current and valid CA RN License orvalid RN license(s) from other state(s). Preferred licensure from acompact state. If assigned to another state, you must maintain anactive, unrestricted RN license in assigned state(s) or the ability toobtain required RN license (in addition to primary state license) within90 days of hire Bachelor of Science in Nursing or advanced degreepreferred Certified Case Manager (CCM) Certification or is in process ofcompleting certification when eligible based on CCM applicationrequirements Requires 5 years of experience in nursing, healthcare, orrelated field A minimum of 3 years of managed care experience ininpatient, outpatient, or managed care environment preferred Transitionsof care experience preferred Strong knowledge of healthcare deliverysystems, managed care principles, and care coordination Excellentcommunication skills Experience conducting thorough psychosocialassessments, developing care plans and providing necessary interventionsidentified during assessment with complex client populations that mayhave, among other things, inadequate coping skills, severe emotionaldisorders, developmental disabilities, mental illnesses, and/or unstablehousing Considerable knowledge of a wide range of psychosocialchallenges and familiarity with behavioral health diagnoses andtreatment Proficient with computer programs such as Microsoft Excel,Outlook, Word, and PowerPoint #LI-AP1 External hires must pass abackground check/drug screen.
