As the primary point of contact for complex outpatients, the Myeloma CNC provides continuity of care through nursing assessments, triage, patient education, and care coordination. The clinical nurse coordinator is an integral part of a dedicated multidisciplinary team that collaborates closely to provide the highest level of care for patients.This is a full-time position at our South Lake Union location. The schedule for this position is Monday through Friday from 8:00 am to 4:30 pm.Assesses physical, emotional, social, and spiritual needs and evaluates patients’ adaptation to health changesDevelops a plan of care based on the patients’ disease, symptoms, and response to treatmentEducates patients, families, and caregivers on disease processes, treatment, side effects and symptom managementCoordinates with the interdisciplinary team to ensure continuity of careRefers patients, families, and caregivers as appropriate to other clinical resources/professionals such as social work, home health care, clinical nutrition, etc.Implements, evaluates, and modifies plan of careIntegrates research and evidence-based knowledge into clinical practiceIn addition to the primary responsibilities of the Registered Nurses (RN), the Clinical Nurse Coordinator (CNC) will have the following key responsibilities:Direct Patient CareEducating patients and families: specific to rehabilitation servicesTelephone and email symptom triageEngage in meaningful patient follow-up and care coordinationex: post-procedure, medication side effectsNew patient orientation to the service lineCaregiver supportAssist with in-clinic procedures:(set up, consent, time out, and patient positioning)Assessment of needs for additional supportive care resourcesPrescription refills and addressing medication questionsResult reviewsIndirect Patient CareSupportive care referralsFMLA paperwork, disability paperwork, Ferry passesCoordination of outside clinical careEnsuring continuity of care with outside providersCollaboration with the research team for patients on trialPlace physiatry return orders and Supportive Care referral orders as directed by MDCoordinate responses to patient inquiries via MyChart and phoneCoordinate lab tests and notification of patientsCoordinate with Supportive Care and medical care teams as needed, across Fred Hutch sitesFacilitate external referrals for ongoing rehabilitation needsManage Epic Rehab Medicine Clinical Support InBasket inquiries from patients, staff, or other providersCommunication and TeamworkAttend regular team meetings with the provider and nursing teamsCoordination of care with ancillary servicesDelegation of tasks to appropriate team membersCoordinating patient schedules with the clinic teamCollaboration with infusion RN and other members of the care team to ensure patient needs are metReal-time management and escalation of issues in the clinicProfessional Development and Clinic EngagementDocument all nursing care activities accurately and in a timely fashion.Demonstrate knowledge and understanding of patient privacy rights.Active participation in continuing education, departmental projects, and committees.Required:Graduated from an accredited school of nursingMust have a current Washington State Registered Nurse (RN) license or current NLC multistate license (MSL); and must upgrade to an MSL with primary state of residence, WA, within 60 days of hire. This requirement may be waived under specific circumstances, subject to approval.Applicants that do not have previous oncology experience will be provided additional training and supportCurrent BLS AHA Health Provider Card (or equivalent course, such as by the American Red Cross) and renewal required every two yearsAdditional certification may be required dependent upon the department.Critical thinking: the ability to practice the cognitive skills of analyzing, applying standards, discriminating, information seeking, logical reasoning, predicting, and transforming knowledgeAbility to develop clinical judgment.Time Management skills: the ability to organize and manage time and tasks independentlyAbility to communicate effectively by looking and listening for cues, asking open-ended questions, exploring cues, using pauses, screening responses, and clarifying responses.Ability to effectively listen by using reflecting, acknowledging, summarizing, empathizing, and paraphrasing skills.Ability to effectively provide information by checking what information the person knows already, giving small amounts of information at a time, using clear terms and avoiding jargon, avoiding detail unless it is requested, checking for understanding, and pausing and waiting for a response.Preferred:BSN-prepared nurseOCN (Oncology Certified Nurse) or BMT RN2 years of oncology nursing experience Previous telephone triage experience Previous outpatient clinic experience Takes initiativeGood communication skillsSkilled at multitasking and organizationThe hourly pay range for this position is from $47.64 to $88.19, and pay offered will be based on experience and qualifications. Registered Nurses are paid on a step-scale basis and are also eligible for shift differentials and other pay premiums such as advanced degree, preceptor, and certification.?Although Fred Hutch is not sponsoring most H-1B visas at this time, candidates who already hold an H-1B sponsored by another organization and are currently in the U.S. may be eligible for this position.This position may be eligible for relocation assistance.This is a full-time position at our South Lake Union location.
