Job Title: Medical Assistant - Population HealthJob Description The Population Health Medical Assistant plays a crucial role in supporting primary care teams within the network to enhance compliance with population health initiatives. This position involves executing standing orders for patients but does not include direct patient care. The role is vital in improving compliance on initiatives such as annual wellness visits, quality care gap closure, and in-network referrals. Responsibilities + Review patient charts regularly to address screening needs, quality gap closures, and care coordination requirements. + Execute standing orders under appropriate physician licensure for screening and quality gap closure needs. + Conduct chart preparation prior to office visits to identify open care gaps for value-based lives. + Manage in-network referrals and conduct member outreach to schedule visits for wellness campaigns. + Coordinate follow-up visits for transitions of care for value-based lives. + Transfer patient data regarding quality care gaps to population health management platforms. + Proactively work on payer care gap and high-risk reports for value-based lives. + Collaborate with primary care teams and clinical performance consultants to manage value-based lives effectively. + Engage in cooperative work with team members in physician offices and support teams as necessary. + Possess a basic understanding of ALOS, readmission rates, and gaps in care. + Perform other duties as assigned by the supervisor. Essential Skills + Proficiency in electronic health records and appointment scheduling. + Experience with EMR systems. + Bilingual in English and Spanish for Central Valley position. + Medical assisting certification (CMA). Additional Skills & Qualifications + At least three years of experience as a Medical Office Referral Coordinator, Medical Assistant, or in a related role. + Diploma from a Medical Assistant program. + Familiarity with CPT codes, especially CPT-2 codes. + Experience with Microsoft Office Suite, including Outlook, Excel, and Calendar. + Knowledge of quality measures such as care gap closures, pre-visit planning, and HEDIS measures. + Ability to follow instructions, analyze data, and coordinate tasks efficiently. + Strong verbal communication skills. Work Environment The position will be based out of a central hub, with the flexibility to float to different provider offices across Maricopa County. The schedule includes working from the office in Arcadia on Mondays, from clinics in the territory from Tuesday to Thursday, and possibly working at a clinic or from home on Fridays depending on clinic hours. This role offers an opportunity to develop new programs within the network and help close gaps in care for network clinics. Job Type & Location This is a Contract to Hire position based out of Phoenix, AZ. Pay and Benefits The pay range for this position is $22.00 - $28.00/hr.
