Summary This Registered Nurse Outpatient & Inpatient Case Management position is in Nursing Operations at the West Haven VA Healthcare Systems, located at the West Haven, CT. campus. This position is a full-time position at 40 hours per week. Responsibilities This Registered Nurse position is located in Outpatient & Inpatient Case Management Nursing Operations located at the West Haven, CT campus. The Registered Nurse (RN) is responsible for providing competent, evidence-based care to assigned patients. The Registered Nurse assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. The duties include but are not limited to: Provides patient management, care coordination, and discharge/disposition planning for inpatient and outpatient settings Coordinates care with multiple providers across all levels and sites of care to include but is not limited to: acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems. Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. Evaluates care and outcomes to ensure timely and appropriate provision of services. Demonstrates leadership in delivering and improving holistic care through collaborative strategies with others. Financial Disclosure Report: Not required Notifications: This position is not a Bargaining Unit position. Selectee may be required to work at any VA Connecticut Healthcare System campus, as needed. Current and former Federal employees must submit copies of their most recent SF-50 (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held. This position is in the Excepted Service. The salary is based on the grade and step approved for the selected candidate. Requirements Conditions of Employment U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. All applicants tentatively selected for VA employment in a testing designated position are subject to urinalysis to screen for illegal drug use prior to appointment.
