Essential Responsibilities ' Perform comprehensive spiritual assessmentand implement or coordinate the provision of spiritual counseling andsupport to hospice and palliative care patients and their familiesaccording to the plan of care. ' Develop an individualizedspiritual/pastoral plan of care which demonstrates an effort to work inclose collaboration with local clergy whenever desired by the patientand provide spiritual support as needed and defined by thepatient/family. ' Offer patients/families of different philosophies andreligious backgrounds opportunities to discuss and share their thoughts,feelings, beliefs and values. ' Provide on-call chaplaincy serviceswhen requested. ' Assess patient/family response to care. ' Performoccasional service assignments such as the annual memorial service andmonthly memorial services with staff. ' Participate in patient careconferences, exploring and assessing the spiritual needs ofpatients/families and update care team on patient beliefs. ' Conduct ormake arrangements for funeral or memorial services when requested. 'Develop and maintain a resource group of clergies to whom specificaspects of spiritual care may be delegated. ' Provide educationalprograms for Housecall Providers staff, community clergy, religious andlay representatives as appropriate. ' Meet with individual staffregarding personal spiritual/pastoral issues that may affect theirability to function effectively. ' Maintain current knowledge andunderstanding of predominant religious/spiritual traditions and variousreligious groups in the service area. ' Collaborate as needed withHousecall Providers staff to enhance continuity of care. ' Responsiblefor documentation in the electronic medical records, assuring accuracy,completeness and compliance with regulations, certification standards,legal and ethical standards according to policies and procedures.Hospice ' Manage the provision of bereavement services in accordancewith hospice policies and Federal Hospice and State Hospice regulatoryrequirements. ' Is a core member of the interdisciplinary group (IDG)and actively participates in IDG meetings for development and evaluationof the patient's plan of care. ' Perform duties in compliance withMedicare Condition of Participation. ' Participate in orientation ofnew employees, volunteers or students seeking experience in hospice.Experience and/or Education Required ' Masters degree in theology,divinity, religion, pastoral care, philosophy or health related fieldand minimum 2 units clinical pastoral education (CPE) ' Minimum of 3months' experience in hospice and palliative care, counseling, hospiceprecepting program, hospital chaplaincy or similar setting ' CPRcertification at hire or within 6 months in position ' Valid driver'slicense, acceptable driving record, and automobile liability coverage oraccess to an insured vehicle Preferred ' Minimum 4 or more units ofclinical pastoral education (CPE) ' Board certification or boardcertification in process through the APC, NACC, NAJC, or SCA 'Experience with electronic medical records Knowledge, Skills andAbilities Required Housecall Providers is an equal opportunityemployer.&
