The Long-Term Services and Supports (LTSS) Case Manager is responsible for the assessment, planning, monitoring and referring activities for people with long term care needs including people who are elderly, blind, physically disabled or living with brain or spinal cord injuries and people with significant mental health needs. Fluency in Spanish is preferred.u00a0 Requirements:u00b7u00a0u00a0u00a0u00a0u00a0u00a0u00a0u00a0Bachelor's degree required and/or five years of relevant experience in the field of LTSS.u00b7u00a0u00a0u00a0u00a0u00a0u00a0u00a0u00a0Experience in the field of cognitive disabilities resource coordination, working with children with cognitive disabilities and their families, and/or advocacy strongly preferred.u00b7u00a0u00a0u00a0u00a0u00a0u00a0u00a0u00a0Driving is a requirement. Must have a valid driver's license with an acceptable driving record.u00a0 Cover Letter and Resume is requiredu00a0- 2 full-time positions available at $27.05-$29.75/hr.
