The Eliot at New Rochelle - We are an Assisted Living Facility located in the heart of New Rochelle seeking caring individuals who are interested in joining our team. Home Health Aide or Personal Care Aide Compensation and Schedule: Salary: $18.55 Full Time & Part Time Hours available Home Health Aide or Personal Care Aide Responsibilities: Assists with personal care activities Monitors residents' health, safety and well-being and assists in keeping environment safe for residents, including reports concerns about residents to coworkers. Does personal laundry, strips and makes residents' beds, launders residents' linens and towels, and folds and returns to residents. Encourages socialization among residents, assists with activity program and chaperones activities outside the facility when requested. Accurately documents services provided and residents' functional status and needs in accordance with agency policies and procedures on the day services are rendered. Responds to resident emergencies, provides first aid as needed, arranges for appropriate medical attention and follow up, and accompanies residents to regularly scheduled physician appointments. Participates in the supervisory process with other members of the health care team as required by NYS law. Performs other appropriate duties as assigned by the supervising nurse. Administration - Accurately documents services provided in accordance with agency policy on the day of service Communication - Immediately reports any changes in client’s condition or incidents to RN Supervisor. Establishes and maintains effective communication and a positive relationship with residents/families Maintains the integrity and confidentiality of residents and family information and client records. Customer Service - Adheres to agency dress code and observes personal hygiene practices when working. Demonstrates reliability and punctuality. EOE/M/F/V
