Job Summary:The mission ofSUNY Upstate Medical Universityis to improve the health of the communities we serve through Education, Biomedical Research, and Health Care. Beyond our downtown campus, we provide care through our clinics and offices across the greater Syracuse area. In support of our ongoing growth, we are seeking highly skilled and motivatedAdvanced Practice Providersto join ourConnect Care Clinic Team at SUNY Upstate’s Nappi Buildinglocated at the Downtown Campus. The full-time APP will provide management care of patients recently discharged from the ED and Hospital, as well as acute and primary care to patients in the Outpatient setting. Key Responsibilities: Providing expert level clinical care & coordination of care via a full range of services that will improve quality of patient care. Assess and evaluate patients, order and interpret laboratory tests & diagnostic studies, and arrange referrals, consultations, and therapeutic services. Build and maintain strong relationships with patients, families, referring physicians, and all members of the health care team. As an APP, you will be an integral member of the multidisciplinary healthcare team, demonstrating professionalism, leadership, and a commitment to Upstate’s mission, vision, and values, while adhering to all institutional policies and procedures. Minimum Qualifications: Master’s Degree as a Nurse Practitioner or Physician Assistant. NYS Licensed/Registered NP or PA. Current National Certification consistent to the clinical practice. NYS licensed/registered NP or PA required. Have a NPI number and be Medicare enrolled with a Medicare Identification Number (or PTAN) (or eligible for Medicare Enrollment within 6 months of hire. AHA CPR-BLS Certification required at the time of appointment. Must successfully pass pre-employment and credentialing clearances. Preferred Qualifications: 1 or more years of experience in Primary Care, Urgent Care or Emergency Department setting. Work Days: Flexible Afternoon / Evening hours, Monday - Friday with weekend rotation.
