Summary The Primary Purpose of the Position is: The Clinical Pharmacist Practitioner (CPP) is an advanced practice provider with expertise in providing comprehensive medication therapy management and functions at the highest level of clinical practice to ensure patient centered care and evidence based positive patient outcomes. Responsibilities Major duties include, but not limited to: Clinical Practice 1. Designs, implements, assesses, and monitors therapeutic drug plans to achieve positive patient outcomes through direct and indirect interactions with patients, providers, and staff in assigned areas. Therapeutic medication plans including but not limited to assessments of clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions and intolerances, cost-effectiveness, formulary considerations, antimicrobial stewardship, nutrition support, and therapeutic outcomes, as required. Findings are acted upon based on scope of practice and/or communicated with prescribers and provides appropriate alternatives to current treatment plans, as needed. Demonstrates expertise in the principles of clinical pharmacokinetics and pharmacodynamics and is knowledgeable and able to make dose adjustment recommendations based on objective laboratory findings. 2. Monitors and assesses the outcome of drug therapies with advanced skill, including physical assessment and interpretation of laboratory and other diagnostic procedures. Administrative and Quality Assurance Functions 1. Participates in medication use evaluations activities, audits or quality management monitoring as requested. 2. Gathers data for administrative reports when assigned. Upon request, independently prepares reports or participates in pharmacy projects. Assists in preparing for special accreditations and reviews (e.g., The Joint Commission, Inspector General, etc.,) and meets with reviewing/inspecting officials and pharmacy management. 3. Actively participates on hospital committees or workgroups as assigned. Serves as a liaison between clinical areas of assignment and committee(s). Will bring back information or requests to Service Leadership for further review and/or decision making. 4. Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities as required to ensure appropriate and safe drug utilization. 5. Assists in the completion of newsletters and other written educational materials as assigned. Formulary Management 1. Supports institutional efforts to construct and maintain the use of Veterans Administration (VA) national formulary that promotes safe and cost-effective use of medications. Provides support to the Medical Staff in drug use policy development and disease management, focusing on appropriate, cost-effective use of therapeutic measures. Provides support to the local and VISN P&T committees to assess and evaluate cost-effective medication use and programs. 2. Provides timely and complete evaluation of non-formulary and prior authorization drug requests for appropriateness and compliance with established criteria as assigned. Education and Research 1. Participates in the training and in-servicing of pharmacists, pharmacy technicians, pharmacy students, pharmacy residents, and other medical center personnel as requested. 2. Serves as a preceptor for pharmacy students and pharmacy residents in accordance with established facility needs. Supports student program and residency programs by completing requests for documents to maintain accreditation and or quality improvement efforts including but not limited to creating and updating learning experience descriptions, completing preceptor evaluation materials, participating in student and/or resident teaching and learning activities, completing ASHP documentation, learning experience and rotation evaluations, and providing information for completion of student and resident schedules and calendars. Communication 1. Establishes, develops, and maintains effective working relationships and rapport with Facility Providers, Nursing and other departments as needed. Serves as a source to resolve questions and problems affecting hospital staff members and patients. 2. Expected to communicate in a manner that affords internal and external customers the ability to experience positive exchange and outcomes. Work Schedule: 0730-1600 Monday-Friday.
