Summary POSITION TITLE: Registered Nurse - Mental Health (BHIP) Case Manager/Care Manager (Henrico CBOC) Responsibilities The Mental Health Behavioral Health Interdisciplinary Program (BHIP) RN Case Manager/Care Manager (Henrico CBOC) provides support to the team by providing care coordination and evidence-based care. The BHIP team uses a Collaborative Chronic Care Model to provide access to proactive, comprehensive, Veteran centered care. The RN is will bring a lot of value to the team, performing crucial tasks such as: function in the role of a Mental Health Treatment Coordinator (per policy), triaging walk-in patients, complete clinical reminders, complete suicide screening, screenings-PHQ-9, AUD-C, PCL-5, etc. send letters to Veterans in the program, medication management and education, secure messaging- answer Nursing level calls and coordinate with Providers needing higher level attention, vital signs, weights, EKGs, etc., call enter notes, long-acting injectables when indicated, patient coordination-labs/UDS, participate and/or facilitate huddles, provide cross-coverage for other BHIP Nurses, assist with completing patient forms, obtain records-within facility guidelines, other related tasks as needed. Duties include, but are not limited to: Determine the nature, extent and acuity of Veterans' needs and triages accordingly. Schedule own appts to coordinate face to face encounters re: patient needs. Provide patients with education re: illness, medication and treatment plan. Provide outreach to clients who are non-compliant of may be decompensating. Coordinate with other agencies and makes referrals to provide patients with necessary community services. Refers, consults with and coordinates care with other VA health care providers. Assess patients and provide additional support between visits and providers. Acts as a liaison to Inpatient Psychiatric unit to facilitate continuity of care. Facilitate psycho-education and supportive resources. Clinical reminders (per scope of practice), appointment reminders, track medication delivered to patient by mail. Secure messages-answer and follow-up, as allowed by policy. Assist with completing forms for patients. Receive crisis phone calls, triage needs, and communicate with providers. Coordinate face to face encounters with patients needed for: antipsychotic injections, Vivitrol Injections, ECGs, and Vital Signs including weight check. Administer the MH Injections on a 2-week or 4-week medication regimen and document on CPRS.
