We're hiring a licensed pharmacist for a contract position at a hospital in Anderson, SC. You will use clinical expertise to ensure safe, accurate medication distribution and provide clinical pharmacy services that support patient care and medication safety. Experience with institutional hospital pharmacy operations required, including unit-dose distribution, controlled substance handling, and sterile product preparation.EPIC experience required.Medication reconciliation and discharge counseling experience required, including preparation of discharge medication lists.Clinical experience with antimicrobial stewardship, including interpretation of culture results and making treatment recommendations for UTI, pneumonia, SSTI, and similar infections preferred.Experience with pharmacokinetic dosing management and anticoagulation dosing/monitoring required.Familiarity with federal and state pharmacy laws, JCAHO standards, and hospital formulary management required.Prior experience overseeing inventory management, controlled substance documentation, and monthly unit inspections preferred.Minimum 6 months of pharmacist experience preferred. Assignment Details Facility type: Hospital.Shift: Day (5x8).Duration: 13 week assignment.Must perform clinical pharmacy duties including sterile product preparation, medication dispensing, medication reconciliation, and participation in Pharmacy & Therapeutics and medication use review activities.May be required to support multiple departments within the hospital as operational needs dictate. Standard Job Description Under general supervision of the Director. Embrace organizational values that prioritize quality, teamwork, resourcefulness, and safety. Maintain adequate drug supply and follow procurement procedures, including emergency procurement and annual inventory processes. Comply with federal and state pharmacy laws and oversee removal and documentation of outdated and recalled medications. Ensure availability of antidotes and emergency drugs in the pharmacy and patient care areas. Participate in development and updating of hospital formulary and provide formulary information to clinical staff. Prepare, sterilize, label, and document parenteral medications and solutions manufactured in the hospital. Verify competency of personnel preparing high volumes of parenterals. Ensure sterile products are properly labeled with expiration dates. Fill and dispense medications accurately, interpret drug orders, and check orders prepared by supportive personnel. Maintain strict control and accountability for drugs dispensed, including controlled substances. Replace drugs removed from emergency carts and floor stock and manage charges to the Business Office. Maintain pharmacy cleanliness and monitor refrigerator temperatures. Conduct monthly inspections of patient care units and pharmacy areas. Maintain records and signatures of authorized practitioners and DEA numbers. Participate in orientation, training, and education for pharmacy staff and other healthcare professionals. Provide medication counseling at discharge and maintain patient confidentiality. Maintain pharmacy computer systems and billing functions. Participate in quality assurance, medication use review, and Pharmacy & Therapeutics committee activities. Monitor patient drug therapy for contraindications, interactions, allergies, and appropriateness of drug and dose. Provide clinical consultation and support antimicrobial stewardship and pharmacokinetic dosing efforts. Promote continuous quality improvement and support accreditation through timely performance evaluations and staff development. Perform other duties as assigned.
