What is Required? + Graduate of an accredited LVN/LPN Program + Current state license as LPN/LVN + Minimum of one year experience in home care or health care industry preferably a nephrology nurse; discharge planning experience and/or healthcare sales experience + Previous experience in home care, care coordination, case management, or patient education + Familiarity with stages of Chronic Kidney Disease (CKD) and dialysis workflows + Sales, outreach, or clientu2011engagement experience (healthcare preferred) + In-depth knowledge of Medicare/Medicaid (e.g. MCO, MLTC, other payer types, etc.), home care benefits, rules and regulations across multiple states + Ability to work with various computer and data entry systems, including Microsoft Office (Word, Excel, PowerPoint), computer systems and other database software + Strong communication skills; both written and verbal + Bilingual preferred + Must have a dependable vehicle, a valid driveru2019s license, and current auto insurance in accordance with the laws of the State. + Sit, stand, bend, lift and move intermittently and be able to lift at least 25 lbs. + 90% travel to local service locations (assigned to a regional area) You will report to the Manager Business Operations. _Equal Employment Opportunity_ _: We are proud to be an equal opportunity workplace and comply with state and federal affirmative action requirements.
