Care Delivery Manager

A client of Innova Solutions is immediately hiring for a Care Delivery Manager. Position type: Full-time/ Contract Duration: 6+ months Location: Flushing, NY 11354 Shift: 8:30 AM - 5:00 PM As a Care Delivery Manager, you will: Day to Day Responsibilities Support nursing standards of excellence and deliver high-quality care through advanced professional education and effective care management. Key Responsibilities: Oversee development and execution of personalized care plans to improve outcomes. Lead and participate in Interdisciplinary Team (IDT) meetings and Service Delivery Requests (SDR). Manage staffing, resource allocation, and clinical delivery reports. Ensure accurate documentation (enrollment, medical records, incident investigations). Collaborate with clinicians on assessments (UAS) and transitions of care. Coordinate long-term care services across facilities and monitor effectiveness of care plans. Maintain compliance with federal/state regulations and organizational policies. Support audits, quality reviews, and continuous improvement initiatives. Manage staff performance: hiring, onboarding, training, coaching, and evaluations. Act as Director of Patient Services (DPS) for LHCSA when assigned. The ideal candidate will have: Required Skills & Qualifications RN license in New York State (BSN preferred; ASN acceptable with experience). Minimum 2+ years management experience in home health, long-term care, or acute care. 1+ year working with frail/elderly populations (or training provided). Supervisory and customer service experience required. Completion of NYSED-approved infection control coursework and required contact hours. Case Management Certification recommended within 2 years.
