At Heritage Communities, you're more than your title -- you're a vital part of a team that truly values every contribution.Whether you're just getting started or looking to grow in your career, we offer a workplace where you're supported, appreciated, and empowered to thrive as you improve the lives of others in our senior living communities.What You'll Do: As a PRN Health Services Coordinator (LPN or RN), you'll play a hands-on role in improving the lives of our residents by: Being a vital part of our Health Services team by working every other weekend and flexibly covering shifts during vacations, guaranteeing uninterrupted compassionate care for our residents.Conducting comprehensive resident assessments prior to move-in, re-entry, and on an ongoing basis to ensure all physical, mental, and social needs are met.Assisting with community readiness for regulatory inspections, including maintaining accurate service plans, medication records, and emergency preparedness.Establishing and fostering positive relationships with residents, families, associates, home health, hospice, and other external partners.Coordinating and ensuring proper health-related training and education for the caregiver team, conducting skill observations to optimize resident care delivery.Providing direct resident assistance as needed, including personal care activities and administering medication in accordance with physician orders.
