Summary The Specialty Medicine Case Management Registered Nurse assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. Responsibilities The VA Midwest Health Care Network advocates for a Whole Health System of care in each of the Medical Centers. This is an approach to healthcare that empowers and equips people to take charge of their health and well-being and live their lives to the fullest. As an employee operating in a Whole Health System of care, you will operate in a model with three core elements, seeking to create a personalized health plan for each Veteran. This is done in the context of healing relationships and healing environments and a connection back to the Veteran's community. This aligns with the Veterans Health Administration (VHA) Mission Statement to Honor America's Veterans by providing exceptional health care that improves their health and well-being. The Specialty Medicine Case Management Registered Nurse duties include, but are not limited to, the following: Coordinates care with multiple providers across all levels and sites of care. Addresses psychosocial, as well as nursing and medical needs of patients and their families/caregivers, through participation in interdisciplinary patient care management practice. Evaluates care and outcomes to ensure timely and appropriate provision of services. The nurse brings principles and theory with limited application of critical thinking skills. Assists Veterans, family members, and caregivers with receiving the most appropriate options and services to meet their complex health care needs. This includes, but is not limited to, acute, chronic, multiple, complex, catastrophic, or life-threatening illnesses; combat stress, residuals of traumatic brain injury; community adjustment; addictions and other health problems Assumes responsibility for the coordination of care focused on patient education, self-management, and customer satisfaction throughout the continuum of care. This position potentially requires flexibility in schedule and assignments. Staffing the clinic(s) will be necessary and expected. Preferred Experience: 3-5 years of clinical experience in dermatology or wound clinics. Clinical experience in any of these clinics would be also be beneficial: Urology, Infusion Clinic, Pulmonary, Podiatry and Rheumatology preferred.
